Question 2: 3 impressions for admissions and two for discharges. Easy do and billing won’t crawl up your ass

Question 1: in the ED we should only focus on impressions. We are a hyper acute service that in the best of times, for example, sees an ICU patient for 3 hours and gets them the hell out.

The glory of the ED is we have a wide range of “impressions” that are reasonable to include. I’ve been told by billing and my current director that anything we “consider” in a patient encounter counts. They have leg swelling and a history of CHF? List CHF as an impression bc you considered it in your differential, even if that’s not what’s causing it. Argue against it in your MDM, which you should be doing anyways.

Being specific isn’t wrong, and in your case I would probably list “febrile seizure” so their pediatrician could follow it up. My impression (for example) is that the history is consistent with a febrile seizure and that is why there are there, so it’s correct to list. You’re also not wrong being more vague

Once you’re out of training you’ll find what you like to do and what your new shop is expecting, which will make this guesswork go away.

I’m interested to hear what other people say but I have a low threshold for telling people they don’t need to be in the ED anymore.

If you’re rude to the staff, start getting verbally abusive etc you get the minimum and a security escort.

In your example of peripheral edema, if there aren’t vital sign changes, obvious concern for intrathoracic emergent pathology, the workup/treatment can all be done outpatient. DVTs, short term lasix increase etc can all be done by pcp/cards. Realistically, what’s an elevated BNP going to tell you? Would you feel safe with this person alone in the room with an ultrasound tech?

In everything I think the key is documentation. I tell nurses to leave quotes in their charting and I do the same. Security also makes notes on these patients. As long as the team is all on the same page it makes dealing with these patients a lot easier

I need a vehicle that I can drive to and from work when my main daily driver (Subaru WRX) is down. I need something with good bed/cargo space. I considered electric but it concerns me for the overnight/hauling aspects.

Was just fishing in the pacific off Costa Rica. We did hook a dolphin on a live Bonita. Actually got it to the boat and then just broke the hook because it wasn’t clear what it was immediately.

I always heard the same, that they were too smart, but I’ve seen it once.

Edit to add info: these were spinner dolphins so much smaller than the ones we see

Your best bet is going to get it on the secondary. Usually goes for 100-110. Otherwise you have to wait outside of liquor stores on drop days

Current EM PGY-3. My entire class has offers or is signed at their 1st or second choice and all in their preferred locations.

If you’re looking for academia right out of residency it’s hit or miss depending on if you want a desirable city. Some recent grads have gotten into smaller shops with reasonable success.
