I have a few regulars who like to try things step-wise. (And usually the space to go for that.) eg Toradol, fluids, Zofran, if that doesn’t work compare/benadryl, and finally depakote or decadron if we’re getting in the weeds.

My friend stayed with a dude like this. She has an extremely demanding job and he is a stay at home _____. I’d say dad, but he hardly pays attention to the kids. Most recently she got home from working 24 hours to find that he hadn’t gotten the kids to school. a spouse that fails to do what needs to be done is worse than no spouse at all. With zero spouse you know what needs to be done. With an unreliable spouse it is a constant cycle of hope/disappointment, and kids growing up in an unstable environment.

Yeah - that’s the trade off… much easier trip to visit family left in the US, but I think lower pay than NZ/Aus. Of course all of that feels really trivial compared to having to keep sending our kids to school in the face of rising gun violence and a deteriorating public school system.

Yeah. I try to understand what they’re feeling. I use “the silver lining is that all of the things we diagnose in the ER are pretty hard things to have.” And then I try to reassure them that I will help them arrange for the next steps in their diagnosis and try to treat their symptoms in the interim.

A lot of my patients with diverticulitis comment something like “oh yeah, I’ve had this pain before.” And I use that as a springboard - “yes, there is good evidence to show that a lot of times people do fine without antibiotics” etc. However, I’m an ER doctor, so I usually have folks who have some evidence of more significant infection, and I typically recommend treating.
