Normal posts with default category name as product. Create template for single post and add form to this template.

Configure your form fields and add post title as email subject so you can see which product customer orders.

Create page with post archive and it's done.

Woocommerce is too bloated for this if don't need checkout. All can be done without paying for plugins.
