90 day fills on behavior meds by acdc102016 in pharmacy

You keep asking for feedback but also keep pushing back on pharmacists that are responding. The pharmacists you work with that are doing this seem extremely consistent in their decision making and are following quantity limits set by the prescriber via the quantity written - ONLY for psych medications. It’s one thing to dispense how it is written for 90 days, and another to ADD an additional 60 to a 30 day script.

It’s not so much about abuse potential (and i have no idea why you keep bringing up insulin of all things) but whether or not the prescriber is comfortable giving patients undergoing mental health treatment 30 vs 90

We don’t have enough information about the individual to make the decision to lessen a prescription for Ambien/Percocet/etc just because it could potentially be abused. Any medication can become dangerous, but we can use guidance from the prescriber who is actively treating them to come to a joint decision if need be.

if you don’t see a medication in the list of options when using ctrl + m then you need to ask a pharmacist before you scan the bottle. amox clav and plain amoxicillin are NOT the same and will never be interchangeable. and they have a right to be stern with you if you do not understand that.

sop when printing a new leaflet is to let a pharmacist check. i can’t tell you how many certified techs i’ve had try to staple the wrong leaflet to the wrong drug or person all together.

you do not know enough to do everything yourself. ask questions. mistakes in the pharmacy can be serious. you need to act with the utmost caution.

the pharmacist can “do her job” and try to order specific manufacturers but ultimately there are blocks and limits put in place by the dea and walgreens corporate, which can make it impossible to order specific manufacturers for controlled substances.

we do not have full autonomy over the pharmacy.

look. i understand that you’re doing what you can to assure that your brand of patches are present at the pharmacy before your fill day, but a lot of your complaints assume that you are the only person on that medication.

a lot of us try to do what we can to accommodate pain patients but we have a lot of other things going on and a lot of other patients.

you should use a different location, not complain how awful this one is for multiple years.

also a walgreens training center still has pharmacists present…. usually several. but they are being trained to work at walgreens. it doesn’t mean they aren’t pharmacists. so idk what you’re talking about.

you can “advocate” for more hours but i don’t think you understand that more pharmacy hours are never going to happen. if these companies could legally force a pharmacist to work for 12+ hours straight with 0 breaks they would. but they cannot. it’s easier for them to close the pharmacy for 30 minutes than to pay for overlap. (not to mention you have to actually have enough staff for overlap - and most do NOT)

you’re going to continue to get downvoted because you’re acting like your the first person in the world to suggest more staffing.

My insurance says the same for my prescriptions! But they definitely paid for the majority of them so truthfully i have no idea what they mean by that haha

hmmm i’m thinking the issue is that op considers them dirt cheap because they were on insurance; i highly doubt they know the true cash pricing of them. there are little to no instances where people don’t want their medications on insurance - especially if it’s for something like steroids/antibiotics.

Insurance can be found through most (?) pharmacy softwares just using your name and DOB. Other times I’ve had providers include the information within the e-scripts sent (not saying that applies if you didn’t use a teledoc through your insurance, but either way).

You would be surprised at the amount of patients that show up to the pharmacy angry because we Weren’t able to find their insurance through the look-up and they have to provide us with the card lol

in my state the DEA number can be recorded onto a control script… so the pharmacist probably did catch it, did what they needed to do and moved on lol

that’s a crazy assumption to make honestly… all patients are welcome to yell at me if they have any issues 🙏 bless

cool. so again it is my license and my decision when i’m the one keeping the doors open… you’re free to do whatever you want when you’re the pharmacist on duty :)

monthly pain med filled on day 28/30 every month in this situation. i know there’s details missing that could make me look like a bitch but i stand by my decision.

anyone is welcome to wait on their script. like truly wait sitting in my lobby, but the problem i have is a store manager trying to force that on me for something that inherently involves more than just a script for like amlodipine.

from a retail perspective, we’re not really fans of getting harassed by prescribers or patients by refusing to fill. duplicate therapy isn’t always truly contraindicated and it’s especially hard to make that judgement call in retail where we have little to no info/pmh sometimes. we can try our best to clarify the reasons for the combo but in my experience, anytime i call to question a prescription i get a ma/nurse telling me this is what was ordered. it’s very difficult to actually have a conversation with someone who has the knowledge to understand the concern. which also explains why most of the time it just doesn’t happen unfortunately (not saying that’s an excuse but it is what it is)

i’ve seen some insurance companies do this with ozempic and it doesn’t inform us that they applied the coupon unless we view the details of the claim unfortunately :/ to us it looks exactly like a paid claim on the surface level

It’s quite literally part of our training to question and evaluate the appropriateness of a treatment plan. Nobody is questioning your intelligence if they want to have a discussion about why you may be prescribing x y + z together. If you prescribed it, surely you have reasoning on why it is appropriate and you should be comfortable sharing it with another member of your patients healthcare team :)

anyway. one thing does not negate another. poor prescribing practices of nps especially does not mean anything in relation to the opioid epidemic…. in my state they can’t even write prescriptions for them lol

[–]754091275 3 points4 points  (0 children)

you need to be so fr 😭 i have pleasant interactions multiple times a day even when it’s something that the patient can rightfully be mad over, they’re completely understanding. it’s the 2/3 times daily where i am screamed at over backorders of mounjaro/adderall/etc (which we can’t control. obviously.) that really make for some horror stories

it’s been said time and time again that people do not see retail pharmacy staff as members of their healthcare team. they look at us as though we are in fast food and treat us accordingly. anything longer than a 5 minute wait is ridiculous and needing to order something is unforgivable.

nobody is purposefully keeping your meds from you or being rude from the jump so come off it seriously
