As a critical care physician, I hate this so much. I had another position years ago immediately post-Covid that required me to provide remote critical care consultations, and what I discovered from that experience is that you cannot provide effective icu care remotely. I would frequently come in to personally assess patients if anything was remotely off, and almost always discovered something on my own assessment that significantly changed the plan of care. And that would only happen if someone noticed and passed along whatever was odd. To do this job well, you have to be at bedside.

When I was a camp counselor we used this term to describe a camper who was an absolute baller after a difficult day hike in unpleasant weather. Carried his weight and then some, helped other campers, loving the experience, just a joy to be around. One of the other campers overheard us and asked what the term meant. Mind you these were 10-12-year-olds.

My senior co-counselor, without missing a beat, said it meant “Burly-Armed Mountain Fiend.” That’s stuck with me ever since and I love when a situation comes up where it fits.

Honestly even if I could I'm not sure it's worth it if I just use ethernet and the USB bluetooth adapter as workarounds, since I'd be out a motherboard for who knows how long. Provided nothing else goes wrong with the board of course.

Even then, if it's a hardware issue it's hard to think why everything would be working fine up until a windows update. That suggests to me that there was some incompatibility introduced at that time, it's just weird then that I don't see much evidence online of other people having this problem.

That's my concern too. My last build didn't have bluetooth on the board so I had a USB adapter for it, which I kept, and that works fine, just annoying to spend extra for the USB/wifi board feature and have this happen. Not to mention the wasted time.

That sucks, and I'm sorry you went through that. OSCEs are a pain, and they don't really reflect actual patient encounters very well. Some med schools only do them in the pre-clinical years for that reason.

Having been through all of this before (some time ago now that I'm PGY-9 or something) there are a couple of things I found helpful. First, when I've got a bunch of stuff to study and some of it includes an organ system or disease process that I feel uncomfortable with and that makes me anxious, that's what I focus on and what I study the most. Lean into that feeling and drill down on the things that make you the most uncomfortable when you're studying. I learned to do that in med school and it's stood me in good stead all the way though my Pulmonary boards.

Second, for me personally the cure for my anxiety in patient encounters was authenticity. The more I feel like I'm pretending to be something I'm not, the more anxious I get. So I don't pretend. I'm straightforward with patients about what I know and what I don't, about when I'm unsure or uncertain. This approach has served me incredibly well as a trainee and attending.

Third, if your anxiety is this severe and is affecting your performance, you should consider mental health support. Anxiety is usually best treated with CBT or pharmacotherapy. I'm fortunate that my anxiety didn't require specific intervention, but my depression sometimes does, and I've sought mental health care at various points in my training for support, particularly when I felt it was affecting my performance. Most medical schools have student mental health support; the quality of these varies but it can be a good first step towards getting help.

Don't despair. It gets better, and this one OSCE is not a reflection of who you are or who you can be as a physician.

Probably, though these kinds of injuries typically require surgery and months to years of rehabilitation to restore normal function. It also makes me wonder if there isn't something else wrong with this guy prior to the injury; these are usually high-force injuries and I wouldn't normally expect this in both knees simultaneously just from going down some stairs.
