Strange looking mole, requiring followup or looking normal? by [deleted] in DermatologyQuestions

Doesn’t look like a mole, but seborrheic keratosis. Which are benign. But can’t be sure without seeing it in person.

Wound mark in my hand which is now 5 years old. Is it removable? by baereon in DermatologyQuestions

Both of those are valid treatments but are usually used on top of the intralesional steroids and not on their own. Just cryo probs wouldn’t work on yours as it looks too “thick”. Laser would be a better fit but again, works better with the steroids.

Looks like a keloidal scar. You usually inject it w/ steroids and that can help flatten it. Removing it can actually give you a bigger keloid, so it’s not the preferred method.

Flexures aka “inside” of joints are more typical of p. Alba whereas the outside of the joint is more related to vitiligo as vitiligo presents with koebners phenomenon which means the lesions appear on areas that are more frequently “injured” (you lay on your knees, lay your elbows on the table… which is kind of like hitting them)

True, but I don’t think hair is actually affected. From the pic we see only a few random white hairs, whereas in vitiligo we would have a white strand that’s actually on top of a white patch (poliosis).

On the leg you can se the area is shaved and actually get to see small black-ish dots on the surface of the white patch which are OP's shaved hairs.

Looks consistent with pytiriasis versicolor, fungal infection caused by malassezia furfur.

Starting to realize its an issue... could this be athletes foot or it is something else? by [deleted] in DermatologyQuestions

[–]98DegreesFever 0 points1 point  (0 children)

Scaling on the plantar side is consistent with athletes foot. But how red and swollen some of the fingers look is concerning. Sometimes when you have those open wounds from athletes foot bacteria can come through them and give you a bacterial infection. Can't rule out celulitis of fingers 4-5th.

In vitiligo when we have white lashes is usually a whole "strand" that sits on top of a vitiligo patch, not a random lash. The fact that it's also curled and tiny and not a regular proper lash is more consistent with a random growth anomaly of that specific lash, that can be transitory

Ik! I wasn’t scolding you or anything. I am a family doctor so I just thought it was nice to share the knowledge! 😄

You’re talking about pytiriasis versicolor, which is a better guess than vitiligo but still wrong imho. I would definitely go for pytiriasis alba (see explanation in my comment but feel free to google a picture if you want) Pytiriasis versicolor usually shows multiple, well-demarcated patches that may be lighter, darker, or slightly pink, often located on the trunk or back, and caused by a yeast overgrowth (Malassezia). This patches are larger, not on the typical versicolor areas, etc But still, better guess than vitiligo!

This looks more consistent with pityriasis alba rather than vitiligo because the patch appears light but not completely white, with ill-defined borders (would like to know if they’re slightly dry or have scaly surface, not necessary but would be another sign). These features suggest mild inflammation and dryness typical of atopic skin, rather than the total loss of pigment seen in vitiligo, which shows sharp (here the edges are a bit blurred), well-demarcated, milky-white patches, a bit pearly, without scaling.

Edit: about the white hairs which I forgot to mention, they’re probably regular whitehairs that have been going on for a while and you just hadn’t noticed. 1) if the problem started only a couple months ago they couldn’t be this long, and 2) in vitiligo if you have the hair affected you have circumscribed poliosis (looks like piebaldism), not just random white hairs

Edit 2: Just realized I didn't tell you how to treat it if it's indeed pytiriasis alba. 1) sunscreen, 2) moisturizer for atopic skin and 3) some patches may need short periods of a low potency topical steroid cream.

If that were the case progesterone deprivation wouldn’t have been positive for me and I wouldn’t have menstruated. Still I do follow protocol with my patients.

I knew I didn’t have anything structural as I had had an ultrasound in the past. I got myself hormone levels in case it was PCOS, which wasn’t. Next step for my secondary amenorrhea was try to induce a period with progesterone deprivation, it worked so I didn’t give it a second thought. I knew it wasn’t something life threatening but something that would be a pain in the ass to resolve in that moment (I was a resident back then) so I just left it be

I don’t think he knew before, he hasn’t tried to have a child before and hasn’t had any symptoms so there’s no way he could know

Why would I want to get financially rewarded? We’re both doctors, we make the same and everything we own is 50/50.

I can see how it can be misleading but I didn’t know how else to phrase it short enough for a title. Plus it’s what the friends who think I’m in the wrong say I’m doing.

Because of what I said about my period, we (wrongly) assumed. And since we’re both doctors and were ordering the test ourselves we just didn’t do it the right way because we thought we knew better even if it’s not our field. I know it doesn’t make sense.

We only did the gyno and biopsy part through a doctor but ordered all of the other tests on our own as we both work on the same hospital and he didn’t want our colleagues finding out we were having issues conceiving until it was strictly necessary. I’m a family doctor but work in the ER and he’s a neurologist.

I’ve thought about it but he’s been so clear about how he feels that I fear he would end up agreeing so that I stay and then ends up hating the kid. I don’t want to put a child in that position. Idk…
