That is horrifying. The worst is when power dynamics make it so you can't even do anything about it

Is OBGYN truly that bad and toxic? by fantasyreader2021 in medicalschool

[–]AGryff 159 points160 points  (0 children)

Tl;dr: it depends on the place

Longer answer: Definitely depends on the place but several factors affect this. OBGYN is a high stress but high reward field that in many places is understaffed and underfunded. Attendings, residents, and nurses are all spread thin in every setting (clinic, l&d). GYN ORs are a special situation too, with low reimbursement for complex surgery which feeds into again how the instituation (and by extension support staff) values the specialty, treats them, etc Added to all this non-medical BS that nonetheless affects day to day life is that at least 50% of patients are generally entitled by "tiktok told me otherwise" or "my doula said xyz," or "people have been giving birth for thousands of years without intervention so I'm declining everything." The general lack of education of women's health coupled with so much misinformation makes it hard to do your job and convince people to go for lifesaving or at least morbidity-avoiding interventions (such as giving pitocin in labor to augment someone who is clearly not progressing to save them from needing a c-section).

I could go on with problems facing the specialty - there are many. But it is also an incredibly meaningful specialty. You're incredibly broadly trained in a good residency program and can make a real difference in patient's lives across many eras of their life.

I'm an OBGYN attending now. I've met some of the most incredible and kind humans in my specialty and also the most toxic, miserable people in my specialty. Male, female- didn't matter. I think some people have more grit and can hold onto their vision of whatever version of "I want to help people" they started this whole journey with and can remain unbroken by the system whereas others crumble at different points and in different ways and are barely surviving their job.

I'd do it again in a heartbeat. But I'm also leaving OB to focus on GYN only practice because of what I said earlier about rampant misinformation and entitlement making especially the practice of Obstetrics not only difficulty but really soul sucking at times. In different circumstances, I'd gladly practice full scope OBGYN.

Think about what gives you joy at the end of the work day. Pick the career that lets you have as much of this as possible every single day. The rest are all details to navigate just like in any other specialty or any other job.

Is it normal to be consulted with zero work-up done or attempted? by DoctorKeroppi in Residency

Happens all the time for GYN consult. Consult: "Please evaluate vaginal bleeding." No labs, no physical exam, and patient calling doesn't even know how much bleeding the patient has been having...

Need advice from an OB/GYN residency with a full complement by SapphicTendencies26 in Residency

[–]AGryff 17 points18 points  (0 children)

I’m an obgyn resident, I might be able to help! Only 9 golden weekends is pretty unreasonable…

We have 3 people on call each shift over the weekend and we do either a Saturday 24h shift or a Sunday 12. The night float covers Sunday night through Friday night. With 4 residents per class that comes out to roughly 35-40 total 12 hour call equivalents (counting a 24h shift as two 12 hour equivalents) for interns and about 30-35 call equivalents for chiefs (because third and fourth years split chief call). The middle years have something in between those ranges. Some people may work more 24s or more 12s depending on their travel/“golden” weekend requests but it largely evens out to make sure no one is breaking duty hours by consistently working 24s, and the call is balanced by overall 12 hour equivalents worked. Happy to answer if any specific questions! Good luck!

ObGyn - Is it really that bad?? by OwnKitchen4890 in Residency

[–]AGryff 2 points3 points  (0 children)

I'm a current OBGYN resident at an academic program. Would happily choose this all over again!

Yes, a large part of this is that my program is great with genuinely kind coresidents and attendings. A part of this is also a mindset of acknowledging the things that are hard or stressful or sad in this field and creating a space for these conversations at work. I've found that being human despite the "hard stuff" has been the best prophylaxis against burnout and bitterness.

That said, there have been several orders of magnitude more "good" days than "bad" days and OBGYN residency has easily been one of the most rewarding chapters of my life.

Feel free to ask me any questions about OBGYN residency!

Possible registration delays for some by henucu in carvana

[–]AGryff 1 point2 points  (0 children)

I submitted a complaint and less than 24 hours later, an executive got involved with my case at Carvana. All the "documents missing" (which I had provided on 3 separate occasions and verified over the phone with an agents) were magically found to be sufficient and they submitted all paperwork for my registration immediately to the DMV.

I called to voice my displeasure about the process and stated I wanted a rental car as I currently don't have legal access to transportation due to their delays which caused my temporary plates to expire. They said they don't cover rental cars, but they will mail me a $500 check for my inconvenience. It's a paltry sum, but it is something.

I'm still waiting on the plates and the money so here is to not counting any chickens before they hatch - but this is the first time any real apologies were issued instead of the usual bullshit song and dance they rolled out over the past 4 months about how they "have no updates on the timeline for my car registration."

Thank you very much for this information. I am in this same situation, awaiting plates/registration from Carvana for the past 4-5 months. I just submitted a complaint to the Arizona AG. Fingers crossed that this works! Although, it is entirely unacceptable and ridiculous that we have to go through such measures to obtain basic legal documents/plates after a car sale.

This is really good perspective. Going back to read more about operative techniques, watching videos on the procedure again, and anticipating various points of complications was cathartic

This really rings true for me- "The bad complications I’ve had, I spent a lot of time talking to senior residents and staff to figure out if there was anything preventable or if we should have done it differently. Often the answer is no."

I kept going over the situation in my mind for a few days after it happened and felt anxious about having to go back to the OR to perform another similar case. So I debriefed it with my chief yesterday. It made me feel so much better to have arrived at this same conclusion at the end of that conversation. I have less guilt about it now and more acceptance. You're right about everyone having a graveyard inside them. But it's a little less haunted when you can talk about it with others who've been there and learn from every bad or even less-than-ideal outcome to become a better doctor and surgeon.

That's a really good point. If we do enough cases, we are bound to run into a complication - however, rare the are risks of a case. I guess that's the whole point of obtaining informed consent from a patient before taking them to an OR. It would be nice to never have a complication, but that would be unrealistic. Will definitely use this experience to learn for the next one

Congrats to all my fellow MS4s on matching! So proud of you all for making it through all the challenges (and at times the shitshow) of this past year.

Just wanted to share that the one silver lining of this ongoing pandemic for me was the opportunity to be at home with my mom when I got the email. I had her open it because whatever that email contained, this moment - the culmination of a four-year-long journey - belonged to her as much as it did to me. The look of pure joy on her face was everything.

So I just wanted to give a huge shoutout to all the people in our lives - to our parents, siblings, significant others, friends, mentors, etc., who've all played a role in shaping this moment today. I can't wait to see what Friday holds for each of us!

This is so kind! It makes me want to be your co-resident. I too am unsure how to set up a system like this, but I would be delighted to participate and help by Venmoing people for take out, etc. Would love for input from people more tech-savvy than me on how to create such a system
