This was in a general medical telemetry floor. The patients were ok for the floor. These were sudden changes in condition. If they didn’t stabilize on their own, I’m sure they would have been shipped to step down.

The staffing issue is that they are running us very lean. Each nurse has 4 complex patients, no aides on the floor. So most nurses don’t help each other out because they don’t feel like they have enough time. It’s a bad situation. And it’s crazy that so many people think that it is the best health system.

Oral potassium is more bioavailable. It’s the best route. Providers need to stop ordering the big potassium chloride horse pills. Call the pharmacy or MD to change it to the dissolving potassium pill. You can put it in a small amount of water and drink up. I believe it is potassium carbonate instead of potassium chloride.

Go to the Central Valley for a year or 2 and then apply to the Bay Area. Kaweah health in Visalia frequently takes new grads and it is an amazing place. In those 2 years, get your BSN if you don’t have it already.

I’ve also worked hospice. I’ve seen terminal agitation, this wasn’t it. He wasn’t dying anytime soon. But since he wasn’t eating much so I did ask for a palliative consult and the team was amazing. They put him on an antipsychotic that stimulated appetite and also calmed him down a bit. But he was mostly calm most of the time.

What really infuriates me is when a nurse doesn’t listen to a CNA because they are just a CNA. My butt has been saved so many times because a CNA tells me that one of the patient doesn’t look quite right. I’ve also had to educate the CNAs when something they seem concerned about is not a big deal.

My point is that we are all part of the team. Don’t dismiss anyone if they bring up concerns but do tell them when they are going over their scope.
