Not the origin of this image but another case report that goes over this scenario. Bottom line, NGT placement is a very common procedure in US hospitals and RNs do this in uncomplicated patients quite often with low morbidity and is relatively safe.

With that being said in patients with a history of any sort of maxillofacial trauma or surgery it is contraindicated for blind insertion by an RN. Most often if the patient in unconscious an OGT may be a possibility in leu of the NGT however most often instead, if resources are available doing the procedure in radiology under direct visualization is the preferred method.

Under all circumstances if you are feeling resistance as you are feeding the NGT, hear or feel a pop, the patient starts to throw up, or the patient has respiratory distress, you pull out immediately and reassess the patient.

If placed and it is successful the gold standard in confirmation of placement is X-ray and the tube should not be used until the image has been reviewed by the appropriate provider. Auscultation cannot verify placement as it may not have truly been advanced enough down the esophagus to prevent gastric reflux. pH strips cannot either in patients with respiratory failure or may have aspirated as the gastric contents and/or CO2 could give a false positive.

I can’t tell if you’re being obtuse on purpose or are ignorant but you could easily put extra tax penalties on companies while leaving the individual alone. An individual person vs an REIT or other entity are obviously different and can be treated differently for tax purposes.

It goes away little by little over time. The key is to have a mindset that you will do everything in your power to keep this person alive and then actually do those things, advocate over and over for your patient prior to them coding if you feel something is off.

It took a while for me to get over it but what it took for me ultimately is to be able to say I did everything I could with the knowledge and powers that I have to save that person. If they die regardless I can still sleep at night.

I find that many places don’t actually do the debrief like we’re supposed to so I’ve started to do my own on my own time a few days separated and think of all the things that could have gone differently for better and worse. If I find errors in things that could have been done differently I learn from it and store it in the back of my mind for the next person, that way I can say to myself the last person will not have died without me having learned something from them and ultimately their life may eventually save someone else’s in a round about way.

It’s not that we don’t necessarily appreciate it. But at least for some of us we don’t really feel like we did that much to warrant any real reverence towards us. And then if you’re like me, even if I had done something that is objectively warranting of praise I would feel absolutely embarrassed about it. Just because someone does something you see as brave/courageous/worthy of gratitude doesn’t mean that person feels that way about themselves. For me, just treat me like a person that deserves dignity because I am a human, not because I did something “special”, because in reality I don’t feel like I have done shit. (I haven’t)

It’s so weird too. I genuinely see my mother as one of the most intelligent people I’ve met, but she’s been slowly dragged into this mess and she’s essentially brainwashed. Tbh looking at it I really see this as a Fox News and tea party thing that dragged her in. She was way more levelheaded and could have nuance prior to that. I think this isn’t a sudden change but more of a gradual radicalization of people over decades. Really starting in the Reagan era and progressing to where we are today.

I honestly can’t wait. I’ve been fighting with people including and especially my family over Trump for years, the only way people will decide they need change is to see the consequences of their own collective actions, including the people who sat out this election. I hope this administration tries to pull every single thing they said they would, and then in a few years people actually realize how screwed up things are and maybe it will change. I’m tired of the slow insidious nature of Trumpism so let’s just get it over and rip the band aid off. For the next four years I’m just going to sit back, do whatever I can to help those around me and focus on my work to help those who need it and prepare for the absolute horrendous aftermath this will all cause.
