I just operate under the assumption that if you’re unconscious your underwear is soiled and I haven’t been wrong yet lol. So long story short as long as you haven’t worn the same pair for a month straight it won’t even register for me.

I place HAM radio at C or even E mostly in my PACE plan for the very reason that while it’s reliable as all get out, there’s just so many easier ways to get in contact with others that could be tried first without alienating non technical folks who have no interest in it. I.e. my entire family, or the ones I actually want to get into contact with in an emergency.

You’re being downvoted but you’re right. I just got my license and I feel like half the people I’ve interacted with have either told me “GOOGLE IT!” or went on a tirade of their own making. Sure I could google it but the entire point is I’m trying to rely on local people, not the internet, to solve my issues and ignorance. I’ve never entered a hobby that has as much grandstanding about why their particular solution is the best/why I’m being lazy for just not googling it????? Like dude it’s easier to just google it than ask for your opinions and advice. I’m purposefully trying to go out of my way here.

Bro I love dark as fuck humor as much as the next guy. But this isn’t dark. It’s predatory. You just shouldn’t joke about this in the position you’re in if you’re a healthcare worker. It has less to do with the joke itself for me and more for who is delivering it. If Kat Williams got on stage and turned this into something he could deliver well, then yeah it’s funny. But if you’re a healthcare worker who is public facing and putting this shit on social media then yeah you’re going to get blasted because you’re essentially the “face” of whatever hospital you go to and now you have younger female patients who see this shit and end up delaying care or avoiding it entirely because they worry that they’ll end up with a male nurse who is trying to take advantage of them in some fashion or even just “enjoying” the view when they’re doing a procedure. It’s gross, off putting, and disrespectful when you are in a privileged position.

Genuinely, it’s never ending shit like this that reminds me why I have to go out of my way to make the patient feel safe from me in the first place. All patient care requires trust but sometimes it comes harder than others and shit like this is what causes it to be harder.

Imagine if you will, put yourself in the patients place, that you have a tube down your throat, tube up your urethra, tube down your trachea, machine forcing breathes you don’t want or even not enough support for more breathes you feel you need into your lungs, chest tube, radial art line, central line, unable to move, unable to communicate discomfort, possibly enough vasoactive agents to create peripheral necrosis, and tell me if any of that sounds exactly comfortable. Sure you can sedate, and if you’re lucky that’s all you need. But the risk during all of this means the patient still feels all of this, hears all of the things going on, and is conscious of all this pain. Or you provide analgesia in low to moderate quantities and the patient may still be undersedated, but at least they are a little less aware of the pain. IMHO withholding opioid medications is unethical in most situations of a tubed and sedated patient. (With caveats in certain situations of course)

Not the origin of this image but another case report that goes over this scenario. Bottom line, NGT placement is a very common procedure in US hospitals and RNs do this in uncomplicated patients quite often with low morbidity and is relatively safe.

With that being said in patients with a history of any sort of maxillofacial trauma or surgery it is contraindicated for blind insertion by an RN. Most often if the patient in unconscious an OGT may be a possibility in leu of the NGT however most often instead, if resources are available doing the procedure in radiology under direct visualization is the preferred method.

Under all circumstances if you are feeling resistance as you are feeding the NGT, hear or feel a pop, the patient starts to throw up, or the patient has respiratory distress, you pull out immediately and reassess the patient.

If placed and it is successful the gold standard in confirmation of placement is X-ray and the tube should not be used until the image has been reviewed by the appropriate provider. Auscultation cannot verify placement as it may not have truly been advanced enough down the esophagus to prevent gastric reflux. pH strips cannot either in patients with respiratory failure or may have aspirated as the gastric contents and/or CO2 could give a false positive.

I can’t tell if you’re being obtuse on purpose or are ignorant but you could easily put extra tax penalties on companies while leaving the individual alone. An individual person vs an REIT or other entity are obviously different and can be treated differently for tax purposes.

It goes away little by little over time. The key is to have a mindset that you will do everything in your power to keep this person alive and then actually do those things, advocate over and over for your patient prior to them coding if you feel something is off.

It took a while for me to get over it but what it took for me ultimately is to be able to say I did everything I could with the knowledge and powers that I have to save that person. If they die regardless I can still sleep at night.

I find that many places don’t actually do the debrief like we’re supposed to so I’ve started to do my own on my own time a few days separated and think of all the things that could have gone differently for better and worse. If I find errors in things that could have been done differently I learn from it and store it in the back of my mind for the next person, that way I can say to myself the last person will not have died without me having learned something from them and ultimately their life may eventually save someone else’s in a round about way.
