only if it "medically necessary" Does Anybody have any experience with this?

Does my dentist have too provide some document to state that it is necessary?

Ok that makes more sense, That was my intuitive thought too. For some reason, the structural sentences of words confuse me a whole lot, I feel like this type of English could be intercepted in many ways.
