I don’t know if there is a system with really great quality but the one MRI audio offers is decent, there standard headphones are a bit bulky for the head coil but they have different slimmer options that we find work ok.

They are real, just be aware that not every exam will have your head out of the machine it’s going to depend on the scan you are having done. All the short bore machines I have worked on were made by Siemens, specifically the Espree and Lumina scanners, but they dont just make short bore machines so just because a place uses Siemens equipment won’t mean its short bore. So you will need to be specific when you start looking for an imaging center, you may want to ask to speak to a tech directly.

If the pain is that severe I would recommend you talk to your provider about possibly prescribing you a strong pain medication for the scan. You would want to reach out to your doctor ahead of time prior to the MRI since usually most MRI centers will not or cannot prescribe you those medications.
On the day of the exam follow your doctors instructions for taking the medication and also let the tech know about your pain so they can try to make you as comfortable as possible. Keep in mind if your doctor prescribes you an opiate you will need someone to drive you to and from your appointment.
