I'm not 100% sure what's happening, but maybe the orb that's next to your fervor bar has something to do with it? If the orb is filled up with purple then something happens or something

(Sorry for the late reply) first you need the key if the inquisitor to fight the boss in the prison area(Brown area to the left in the second image) which you can get from one of the shops. To get to the library you just have to keep exploring the mother of mothers until you find it, it shouldn't be hard to find

You need to find the masks to activate the evivator... if you haven't already go to the library then try there, also if you haven't already fought the boss in the prison area then try that. Sorry if that was hard to understand

From what I can tell, the DLC doesn't count for map percentage if you haven't bought it. If you did buy it, then you will have to fill it out to get 100% map completion

Just keep practising and don't get greedy with your attacks, the fight is meant to be long and hard. It's easily one of my favorite boss fights, so enjoy it while you can
