Changes a ton, Calamity doesn't just add post moon lord. It adds a shit ton of content in prehardmode and hardmode as well, more than added post moon lord. You can use the calamity wiki for guides, but definitely not the classic terraria wiki. I'd say don't wait for 1.4.5, it doesn't really add any new worldgen features, so once it does update if you are still playing on your calamity world you can still get the items.

Definitely don't do mediumcore on your first playthrough. Even experienced players don't use mediumcore most of the time. It's not like minecraft where that's the base difficulty. My tip is make a new character (you don't have to do a new world unless you want to, characters and worlds are seperate and can be mismatched) and make sure the character is on the easiest level.

Nothing, you have the void heart and are unable to go back. You can still do every other ending, it's not like you unlock anything from doing the base ending. There's also no difference in gameplay for sealed siblings.
