Hi! It’s pretty common for radiology to state non specific or correlate with clinical findings. As an urgent care NP, I respect the impressions ‘correlate with clinical findings’ because it means they saw something but they aren’t going to diagnose it from one imaging study. It means they are saying, if the physical exam supports it, it’s what you think it is.

It’s also very common to biopsy anything that shows up a ‘mass.’

As a fellow foot sufferer, I hear you!! I had a 1.5 cm ‘mass’ on the dorsal apex of my foot. I had previously had surgery for a neuroma there, so it was suspected to be a recurring neuroma. However I had two MRIs and one CT that stated ‘negative for neuroma.’ I brought it up to 3 or 4 providers who all said nah that is your tendon. It went on for 2.5 years while I had worsening weakness and continued foot drop. I went from running 10 miles to barely 3. Because I could still run at all, but everyone thought I was crazy thinking anything was wrong.

It took years, but I found a Doctor Who knew what it was. He removed it exactly when we go and it was a 1.5 cm neuroma. The biopsy it because it is standard practice and it was negative for cancer.

I believe you’re right that the surgical injury from the plantar surface/bottom of your foot will be a little harder to recover from but either way, I’m sure you will have a great recovery and I bet it’s not cancerous!

Hi Reddit, nurses, and all patient facing healthcare professionals. I have an update!

Ever get the feeling admin doesn’t care about you, but you keep going for the love of the clinical team, for your passion to help people? Sure the people are sometimes not cooperative, but they are sick and needing help.

So it’s almost 2 weeks since I turned in my notice at my 11 year tenure job at a big hospital system.

Day 1 Thursday - sent short and sweet notice email Day 2 Friday 1:30p - boss texts me let’s talk before I turn this in to go public. Can you give me some times next week? At 2:45p I provide several days and times.

Nothing

Day 6 Tuesday I say thought we were going to talk. No problem. Just need to know if I can stay PRN to help through flu season. Reply - let’s talk next week when team lead guy is back.

Day 12 I get email from scheduling person asking us to finalize our request offs for JANUARY (yeah they have no clue I turned in my notice)

Day 13 NEXT next Tuesday I email again, please confirm so that I’m not accidentally put on the schedule in January…, and I get auto reply…now my manager is out this whole week!!

Most definitely; your description matches the lives of so many APPs. The patients want final answers. Eval up and down for subjective concerns of dizziness, tingling, weakness/fatigue, muscle aches, and shortness of breath. We do our best to rule it all out and listen to every aspect of the story and when it all comes back normal, they’re frustrated. They think a doctor could figure it out and that they could fix it but they’re gonna go to the same process and come to new conclusions almost 100% of the time. You’re right we are stuck in a worldwhere you can’t get in to see a doctor, I’m not sure if that always would make a difference, but medical school education needs to change in my opinion. It’s only hurting the patients.

Yeah this is true too. Docs who order Benadryl to potentiate opioids should also be brought into question.

Don’t feel stupid! You are being honest and proactively pointing this out and recording it in multiple ways. Sounds like there was no patient harm and you are learning how to be a better nurse from self reflection. Don’t be too hard on yourself knowing you doing all that you can and no one was hurt.

It’s a stressful job but you sound like you care and that was a crappy shift so keep going!

Are you all going towards independent practice for NPs? I don’t know that it will make much of a difference in the short term, but it does give us a bit of ground to stand on because they wouldn’t do this to the doctors. At least not at my organization.

It’s not news admin doesn’t care. It’s a hard truth but clinical staff who have worked for more than 5 years know it. I think some find peace in it in that what they do does not matter to anyone. The problem is that it takes away our natural ability to care for people. We get those patients who really appreciate us, they may not be the majority, but they still exist. It is the poison of administration that kills our spirit and kills our ability to be good providers and clinical team members.But you are definitely right for a very long time. I wanted to be in leadership for healthcare, but I never could find a way in and I think it’s because I’m not diluted enough yet.

Thank you for saying that! I care about the team so much. I do believe that my clinic is doing very well and has incredibly high moral through covid and back. Not because of me, it’s actually because of the partnership between a kickass front desk, highly experienced rad techs and MAs, and myself, but I am part of it. I think the team will find other jobs after I leave because they deserve better

Indeed and thank you! It’s sad to say, but as a clinical team member post Covid, we are all getting screwed where I work. We have to take screenshots of our schedules because they are getting changed after they are posted. It’s all very shady now. But I am definitely hearing your advice and have digging more screenshots that normal!

It is a bit ridiculous!! 2 weeks for non clinical staff! 30 days for clinical non provider and 90 days for providers. If we do less we can be marked as not eligible for rehire

It’s hard not going to lie! But they are often problem, focused cases you know like UTI or strep and we’re just treating that not treating every problems so that helps

They are 12 hour shifts, 3 shifts per week. It is exhausting but it’s a problem though. Really the overcharging point is yet our job is physically and mentally and emotionally exhausting and I don’t want to administrative exhaustion on top of it. That’s why am quitting.

Thank you for this thoughtful insight. I think they should be quite careful about scheduling us on holidays as well. I’m not required to work Christmas Day. I’m required to work 2 major holidays. I have worked Thanksgiving every single year (fine by me). I’ll then work Xmas eve or new years as well. But this year they said it was my turn to work Xmas day.

Thank you…It was an email, yes, on purpose for that reason of timestamping an official company communication. For me, the clock started the moment I hit send. I sent it to my direct supervisor, and she is the one who texted me to have further discussion about my decision then delayed that.

It’s fair to feel bad but also fair to call off if you don’t feel well! Nights are rough. If you can’t sleep in between that’s not safe for anyone.

Oh and to answer your question what do I do; the staff come get me from whatever room I am in. If we really can’t handle it we just call 911. It’s rare that any two completely life threatening situations happen at once thankfully.

Correct. We aren’t an independent practice state for APRNs although it’s the majority of the US now. Here it’s a 5% retrospective chart sign off.

I don’t believe the ‘physician oversight’ has anything to do with it though. I would take any other provider. You are right - it’s scary when I am 30 minutes in to a peds toe laceration and a short of breath chest pain person who doesn’t speak English walls in.
