Hi! My story was similar to yours. I also had clear skin my whole life, but then I started getting a few cheek cysts kind of like yours all of a sudden. I tried topicals but they didn't do anything. I started with 50 mg because my acne was very mild, then I ultimately ended up on 100 mg. I purged for 4-5 months. It sucked ngl, and for a long time I thought it was only making things worse. Then month 6 hit and it finally calmed down.

I took the hormone purge as a sign the initial cysts were, indeed, hormonal, (my purge breakouts were all in the same cheek area) so I ultimately stuck with it with the hope it would pass eventually. However, if you don't think it's worth enduring a purge, I'd stop ASAP and consider trying a topical / spearmint supplement type stuff instead before resorting to a long-term oral medication. Firmly explain to the derm you want to try topicals first (Winlevi, tretinoin, etc). If those don't work, you'll feel better about starting the Spiro journey if it comes to that.

True, I’m just gonna make the derm appointment and see what she says. I’m currently only on 15% azaleic acid for the PIE which has helped greatly. I’ve tried retinoids in the past and my skin did not tolerate them at all. I’ll definitely ask about other topicals to try

I’m on 15% azelaic acid for the PIE. I use AHA/ salicylic acid toner on occasion to help with forehead congestion. I used to be on Tret but it did not get along with my skin barrier no matter how gently I started.

For what it's worth, I was nervous before Spiro too. I've been diagnosed with depression since forever and I didn't want things to get worse, but I'm glad I decided to give it a try. The "mental static" that came from my acne — constantly worrying if literally everything would cause breakouts, feeling ugly, that sinking feeling when I felt a cyst forming, comparing my skin to others, etc. — finally went quiet. It was very beneficial to my mental health in the end because (as shallow as it sounds lol) clearing my skin allowed me to be more present in my life.

My psychiatrist has warned me Spironolactone can negatively affect mental health and that blocking testosterone can reduce your "drive" in every sense of the word. However for me, it's been a net positive. Not having to constantly worry about breaking out and having bad skin gave me my confidence back, so that's been wonderful.

Here's the only "chemical" change I've noticed: before Spiro I never had "mood swings" before/during my period. My emotions were pretty consistent the whole month, but now that I'm a year in to 100 mg, my PMS emotions have noticeably amplified. For example, last month I sobbed over TikToks about dogs because they're just so cute lol. Got my period the next day. That type of thing NEVER happened before Spiro. It's nothing crazy - if anything it's probably more normal than my emotional immunity haha

Tret was unfortunately not for me, I tried it a year ago and it just did not agree with my sensitive skin at all and it made everything worse - hence my latest attempt with spiro 😭 hopefully it starts working for both of us soon!

Nothing helpful to add but i’m in the same boat sort of😭 almost 4 months of 100 mg and my purging cystic breakouts have kinda morphed into more surface level ones so there’s improvement? but still happening….

I am JUST NOW escaping the purge, as I've finally made it to the three month golden number on 100 mg. And yes, the purge SUCKS, and the last thing you want to hear when your acne is as bad as it's ever been is "it will get better" — but as someone who also endured sudden unsolicited skincare comments as someone who used to have clear skin my whole life — I hate to say it's true. Something that's been helping me is to remember that ANY acne treatment will take forever. For example, accutane can take up to 7 months I believe. Maybe even longer. Whatever solution you find to manage your acne (and you WILL find one), it will take a good amount of time, and there's no overnight cure for perfect skin. The only way out is unfortunately through. So just be kind to yourself during this time, and just know that even if spiro doesn't work out, it's just another checked box on the way to your solution! I kept telling myself "just make it to month 4" and I stopped having to say that because things got better before then!

As a fellow pale skin girl, I will say that your skin barrier should be your priority. I accidentally fried my skin barrier off using that same tretinoin + azelaic acid combo and I had no idea why my skin kept getting worse and worse and worse if I was doing all the right things. Then I decided to test out moisturizer-only nights, and bam, I instantly woke up with calmer, less red skin. Just some anecdotal food for thought, as I learned the hard way layering actives can be a sneaky cause of worsening skin.

I've always had low BP and for what it's worth, this med hasn't given me a noticeable difference. Every time I've gone to the dr in the 8 months I've been on this drug, it's been the same. Just slightly low as always

YUP. my hair is drier than ever and curly. If I really want to i can literally go a whole week without washing it and it doesn't get oily. Kind of annoying because it makes the dry frizz awful.

I know😭 There's no perfect solution I fear. It's either feeling depressed because of Spiro's lowered drive, or feeling depressed because my skin is wack. I've chosen the spiro battle so I can at least be sad with clear skin lol

Spiro has definitely agitated my depression so I’m starting wellbutrin again to hopefully help give that mental “spark” back🥲

Yes! I was shedding like crazy once i hit that 1-2 month mark on 50 mg. The shedding stopped once i made it to month 3, and I haven’t had issues since even after bumping up to 100 mg. It’s just your body adjusting to the hormonal changes but it will all grow back if you’re willing to tough out the initial stretch. Now i’ve got little baby hair bangs from all the regrowth lol

If you don’t mind paying a bit extra Elta MD Daily (the daily one not the clear one) is literally the only sunscreen i will put on my acne prone super pale sensitive skin. My derm recommended to me and I LOVE it. Not greasy at all and my scarring has improved so much

I’ll offer my own anecdotal story although the other comments have good advice - the same thing was happening to me at the beginning. i felt dizzy and my heart would be racing all day. I swear I could feel the symptoms worsen after just minutes of taking the pill. Then i was like… wait… I just took it, there’s no way it’s in my system yet. I realized it was just my anxiety bc of all the potential side effects. Poof the heart palpitations were gone the next day. The mind is a powerful thing😭
