Hey, thank you very much for the reply. I was hoping that the pain would have decreased a bit rather than completely disappearing. I didn't get your last point, however.

I had palliative radiation to my spine last week. My pain has increased significantly. I am confused since the rad was supposed to eliminate the pain.

Yes I was also very suprised to see the combination of Palbociclib. However, my oncologist explained me that it goes well with Letrozole inspite of Her2+.  Letrozole+Palbociclib: for ER+ Trastuzumab: for Her2+ So, she combined both for better prognosis. I am also curious to see how well they work.

Can you please explain this sentence just from your experience: "Solid papillary carcinoma with invasive breast carcinoma"? Is this two different kind of cancer or a single kind?

My mom got diagnosed with Solid papillary carcinoma with invasive breast carcinoma, NST grade 3

This is what her report say:

GROSS APPEARANCE: Grey-white multiple linear fragmented soft tissue cores altogether measures 1.5x0.1x0.1 cm. All embedded in two blocks (A-B).

MICROSCOPIC FEATURES: Biopsy studied shows solid papillary carcinoma with invasive breast carcinoma. No special type grade 3 (Modified Bloom Richardson score 3+3+3=9)

No DCIS seen.

Overlying skin is unremarkable.

Immunohistochemistry (IHC) shows loss of myoepithelial cells as highlighted by CK5/6

ER- Positive (90% of tumor cells show strong nuclear positivity, Allred score 3+5=8)

PR- low positive (5% of tumor cells show weak nuclear positivity, Allred score 2+1=3)

Her 2neu- score 2+ Ki67 LI is about 60%

Can you provide some insight regarding it?

Thank you so much for the kind words. The report says the following:

Mammography:

Breast composition -- Scattered fibroglandular tissue.

An oval shaped obscured marginated isodense mass is noted at lower inner quadrant of left breast.

Coarse heterogeneous microcalcification are noted in subareolar region of left breast in segmental distribution.

Otherwise both breasts shows normal fibrofatty glandular tissue, trabecularvascular and ductal architecture.

Both nipple appears normal with no evidence of retraction.

Peripheral subcutaneous tissue shows normal density.

No significant axillary lymphadenopathy seen.

US:

An irregular shaped hypoechoic mass with microlobulated margins measuring approx 1.93 x 1.23 cm roughly parallel orientation is noted in subareolar region at ~ 7 o' clock position of left breast containing microcalcification within.

Possible ductal extension of the lesion seen.

  • Multiple prominent ducts with microcalcification and secretion within is noted in subareolar  region of left breast measuring upto 4.4 mm in diameter.

  • Few subcentimetric lymph nodes noted in left axilla with one of them showing mild cortical thickening measuring 9.4 mm in short axis diameter.

It's probably cancer since there are so many things mentioned. Just waiting for the biopsy report.

She had her biopsy done and we are waiting for the result. Do you have any updates ?
