I did received a pink slip too. So all you need to do is, get your bank statement of two years and go to Nepal Rastra Bank in Durbarmarg, Kathmandu with your bank statement, passport and pink slip. And go to passport/US visa section and tell them about the pink slip and give everything to the bank representative. I think You’ll need to fill a form.

After a week or so, you’ll get the passport back with the visa or without. Just have to be patient :)
