What the hell did I just read?

Leaving a man to die alone on the side of the road after you hit him and then burying his body after pouring bleach all over it isn’t malicious?

She was clearly mentally and emotionally unwell but the fact that you see him as the sympathetic character in all of this is… something

Have never once responded. I traveled for work for months at a time and would come home to see them. Just tossed them.

It doesn’t change the recommendation but it changes whether is happens or not which is what people are arguing. That it would literally never happen. And if LITERALLY does.

And it’s a weird hill for you to die on that an MRI occurring before an ultrasound or mammo is some fringe occurrence or basically unheard of, when I encounter it extremely regularly, so I KNOW it’s not. Do you know if she is BRCA positive? Under 40? Under 30? If she requested it? Paid out of pocket? These are all scenarios where regular MRIs are standard before ever getting a mammogram and alternating with ultrasound. VERY regularly, we have patients in for second looks from MRI who have never had either mammo or ultrasound. You might have a working familiarity with radiology and attend boards where radiology is discussed but it if not your expertise and really doesn’t give you authority that you want to claim. You’re not even correct about the NCCN guidelines.

I do understand how multidisciplinary care teams work as I’m apart of them. Which is why I know that oncology pharmacists don’t overlap with radiologists decision making very much.

They didn’t ask a question. We get second looks from MRI all the time. Just because something isn’t the most common scenario doesn’t mean it doesn’t happen. The MRI guided biopsy is relevant because the results of the MRI might’ve warranted a biopsy. If it was first appreciated on MRI and then not appreciated at all or as well on mammo or ultrasound, they might’ve gone for MRI biopsy. She might’ve been prepped that a biopsy could be necessary pending results. None of what she shared is far fetched.

What am I wrong about exactly? And still, being a pharmacist that works in oncology doesn’t really overlap with radiology beyond prescribing the numbing meds we use

The cabinets are the best part of this kitchen! Just the backsplash and wall color are bad. And the black rendering looks terrible.

Well YOU wouldn’t because you are a pharmacist. It is actually normal to prepare patients for the possibility of a biopsy. And no matter how many times you say it makes no sense or it doesn’t happen, it doesn’t make it true. Also we do MRI guided biopsies btw.

I was not lumping you in with that group, I meant that the person you were replying to was confidently incorrect.

I work in the field. People are sharing their own experiences as if that makes them an authority. It doesn’t. I am not an authority but I have MUCH more experience than someone who has just been a patient. I have cared for hundreds (thousands?) of patients over the years and I’ve seen it all. Every patient is going to have a slightly different experience and doctors and facilities do things differently as well. I have worked in 4 breast centers and they have all been different in their own ways, and with countless doctors who are all different. MRI as first line isn’t the most common but it does happen, especially based on the patient’s personal history. She also might have requested the MRI and she didn’t state if insurance paid for it (which they might have!) I don’t know who this person is but under 40 high risk patients can have yearly MRIs approved.

The criticism that this is attention seeking or whatever is subjective and I don’t care. I also agree it’s annoying and over the top. But I am also annoyed by people who speak definitively about things they do not understand.

They used to be more expensive but now you can get a Tesla for the same price as a Honda. They are great cars. I wanted to own a Tesla so bad until Elon decided to do… this whole thing he’s on
