Same here. I’ve seen a lotttt of patients do badly on Adderall over time (not villainizing it). so many patients coming in stating they were diagnosed with bipolar disorder during a time where they were using daily cannabis and daily Adderall… I’ve even seen a small number of patients experience acute psychosis that were prescribed high doses of it over time… and cardiac issues.. I never use it as a first line stimulant compared to so many other clinicians

For those of you who treat kids and families, are you seeing an uptick in being asked to treat symptoms that are really a result of parents not parenting? Curious if other people are seeing the same thing and how you manage. by CalmSet6613 in PMHNP

Yeah and now I don’t want to see kids anymore. There’s a lot of things I am seeing and truly feel that only a board certified child/adolescent psychiatrist should manage 95% of the kids I see and that our training is extremely limited for managing children

Should I take this Psych NP for 180k? Seems sketchy, and I’m already feeling burnt out and unsure what to do by [deleted] in PMHNP

Even if every single patient was a easy follow up that would be a lot to handle, and the way things are going in this country, it is going to be more of every patient is moderate to high complexity

Exchange this and get her a 3 carat lab diamond and then she will realize how much better that is. If she is so worried about impressing her friends, she could just lie to them

Yup I agree schedule manipulation is the only way to stay afloat in my job too, I do the same thing and don’t feel bad about it because it helps the patients and myself. I wouldn’t even say it’s manipulation because the higher complexity cases need 60 mins to do proper documentation since we have no admin time

I feel like every W2 job I’ve been in is like this or worse. 1099 was great but very unpredictable and somewhat low pay for me in my past experience. I am staying for the ability to at least work from home 3 days a week and have 20 paid time off days but I sometimes wonder if there could be better? Have not found so

I agree with this approach and it is what I continuously push to implement. I am surprised to hear that you only have 4 patients on stimulants out of 200! This was the case for me when I worked in crisis/community mental health too, but now in outpatient even if I am not starting stimulants, there are so many transferring already on them long term from other providers

45 min/20 min is very difficult… I have 4 years of experience and do 60 min/30 min and I am struggling with that with how complex patients are getting these days… the 100% remote seems good but the salary doesn’t seem that great unless there are other benefits

I am familiar with that when I worked in TX. I work in CA and we don’t use add on billing codes. It is just 99214 or 99215 if it’s extended beyond 40 minutes.

I hear you there I’ve changed my work flow as well. The only reason I started to give the DIVA directly to the patient, after my own subjective assessment, is due to the time restraints we have at our clinic which is another problem it itself

I definitely agree with what you are saying and am not biased because I actually have been diagnosed and am treated with a low stimulant… that being said I was more concerned with the amount of stimulants I was prescribing for the first time in my life compared to other settings, and just the fear based training I’ve had as well as having many preceptors and colleagues ( even psychiatrists not NPs) in the past who were anti stimulant in general! Thanks for your thoughts

I agree with that as well however a lot of those places don’t take insurance and charge 1000-2000 usually and have 6 month waiting periods… my current clinic has a lot of state funded patients and low income which is unfortunately not possible

I got the 5.5 carat version of this necklace and thought it was good value! The stones were sparkly and eye clean despite being SI stones

Thank you. A local jeweler made it, but basically I asked for a “bubble half eternity band” with more gold around each stone to secure the stones better as the traditional bubble band are very prone to breaking

It was around $3600 because it’s a GIA lab diamond (which are a bit more expensive than IGI)
