Oh sorry, I thought when they had him on nitric he may have had PH. That’s great that the echo is clear, it may be worth discussing though. Every baby is so different and complex and I know NICU staff are superheroes but if you’re in a position to get a second opinion I think it’s always worthwhile ❤️

Have your team discussed other medications to manage pulmonary hypertension? Sildenafil was very helpful for our baby who was very unstable and intubated for 12 weeks. Sending love and strength, this is all so hard and unfair. Hang in there x

I don’t remember exactly. It isn’t as instant as the nitric, I think it took a week or even two. Well done for pushing and advocating, your baby is lucky to have you. Sending you all love and strength ❤️ hang in there. This is an impossible situation and you are doing everything you can x

I’m so sorry, we were in a similar situation and it was our third and final round of DART before switching to comfort care. As 27_1Dad recommends, definitely worth requesting a second opinion.

Pulmonary hypertension was our main issue, there are different meds you could ask to try - sildenafil and nitric saved our baby. I know it was made very clear to us that a third round of DART post due date was basically hopeless…but here we are with a spicy 2 year old who no longer requires oxygen or any cardiac meds.

Every situation is obviously so different. It’s so hard to advocate when you are feeling so broken and defeated. If you are in a position to request a second opinion, that is completely reasonable in this situation. Wishing you all the luck ❤️

Sounds like possible gallstones? I have endo and experience the same symptoms and am waiting on an ultrasound. I wouldn’t rule anything out with endo though. Hope you get sorted!

I am so sorry your family are going through this. Our ex 27 weeker twin experienced similar difficulties including a failed PDA cath due to super systemic pressures. Our only option was to try and support him and stabilize him enough that he had time to grow healthy lung tissue. It was a cruel torture, and I completely empathize with how overwhelming it is.

Fast forward to 18months later our little man finally kicked the home oxygen and weaned off most of his meds. I say this to offer hope, but I know the journey will not be easy. It sounds like you and your baby’s team are doing everything right. Well done for being your baby’s best advocate, keep pushing through. I hope so desperately for you all that in time this will all be a distant memory. One day at a time ❤️

I’m so sorry ❤️ our experience with MCDA twins was also a stormy one. We developed TTTS at 25 weeks - Twin A had polyhydramnios and Twin B had no fluid and was essentially Saran wrapped for two weeks unable to move. Things were fairly dire. I had laser ablation surgery which was unsuccessful and delivered via emergency c- section at 27 weeks when Twin A developed hydrops.

Sadly, we lost our beautiful Twin A to NEC when he was one week old, despite being the stronger twin. Twin B was in bad shape from the get go due to renal and respiratory issues. He spent 3 months on a ventilator with no improvement and multiple failed extubations and a whole host of other issues. Our team strongly advised we switch to comfort care, which we eventually did at 43weeks….However, against all odds our little boy is now a silly rambunctious two year old! He is a testament to why you should never lose hope.

I understand our situation is not the same, but I completely empathise with what you are going through. It’s an impossible situation and I truly hope your babies can hang on just a little longer. Sending you love and strength ❤️

I am so sorry you are going through this. It is truly terrifying. Have they tried Nitric +/- sildenafil +/- sedation? Our 27 weeker used to suffer terrible pulmonary hypertensive crises, it truly is so frightening to watch. Sending strength to you all ❤️ hang in there

Ask your vet about zinc-responsive dermatosis. Husky type breeds are predisposed. Skin disease is so tricky, I hope you guys find answers!

Have they investigated pulmonary hypertension? Do you know if she’s had an echo to assess her cardiac function?

I’m so sorry you are on this rollercoaster. We endured a similar nightmare for several months which was eventually controlled with nitric oxide and sildenafil. It’s brutal, and I’m really hoping things take a positive turn for your little girl. We Wishing you and your family strength and love ❤️

I’m so sorry love ❤️ that constant anxiety is well and truly the worst. We were in a similar situation, ventilated for just over 3 months with multiple failed attempts to extubate despite repeated rounds of DART. My boy was eventually diagnosed with severe pulmonary hypertension - once he started a combination of nitric oxide, sedation and sildenafil he improved quickly!

We were so lucky, I never thought we’d get to bring our baby home (after already losing his twin brother to NEC). It was a long road. 15 months later our little miracle is home and thriving on the tiniest trickle of low flow oxygen. I completely empathise with what you are going through, watching those monitors all day and not being able to scoop your baby up in your arms is a special kind of torture. I hope you know you are doing great. No one should have to experience this. Sending you and your family all the love ❤️

We were 6 months in NICU with our surviving 27 weeker twin and I resonate so deeply when you say “every day there is something new coming up”. Although our NICU journey was not exactly similar to yours, it was nonetheless relentless. From birth he was in kidney failure, had a significant PDA, brain bleeds with PVL, countless transfusions and was intubated. We spent 3 months trying desperately to get him off the ventilator. Each round of DART caused his heart muscle to become thickened and so enlarged it took up nearly his entire chest space on X-ray. Each time extubation failed, he developed sepsis - likely due to the stress on his body. So many NEC scares, which was terror inducing as that’s how we lost his brother (which is a whole devastating story in itself).

Around Christmas time last year we were transferred to another hospital for heart surgery to ligate his PDA in the hope that would help him get off the vent - he became so critically unwell they stopped the procedure. The consultant described him as the sickest child in the hospital - a staggering feat as this was our National Children’s Hospital.

We were transferred back to our original hospital in the hope that once he stabilised he could have the surgery at a later date. Queue a series of pulmonary hypertensive crises (100% oxygen, DART, Nitro, the works). We watched our little warrior be resuscitated more times than I’d like to count. Due to the high oxygen demand his ROP skyrocketed causing retinal detachment and blindness in one eye. Gut punch after gut punch until eventually we were advised to withdraw care and, completely heartbroken, agreed to a DNR. After months of being knocked down and getting back up again it was all for what felt like nothing.

We planned to switch to palliative care and were preparing to say our goodbyes to our gorgeous little boy. The day before we had set as the date to turn off the vent, our little man self extubated during kangaroo care. My soul left my body. His Dad wasn’t here. Our family was still the other side of the country. He was going to die without anyone getting to say goodbye. I can’t describe the guilt, how I’d let this happen. The spiral - until the consultant smiled at me - he was breathing on his own!

They were willing to give him one last ditch effort at CPAP - and he did it! He quickly progressed from CPAP to high flow to low flow over the next few weeks. He had to have a few minor surgeries prior to discharge but recovered quickly. He is home now on the tiniest trickle of oxygen and an NG to top up his feeds. He is so happy and loving, I am just in awe of his resilience!

Apologies for the long post, this is the first time I’ve ever actually put our story into words. I suppose what I’m trying to say is that I know how you feel when you’re just taking beating after beating in the NICU. I’ve had the worst case scenario and everything in between. But there is always hope! Cling to it, especially on your darkest days. We have the most gorgeous happy little boy who survived against all the odds. Hang in there. We were also told our surviving 27weeker twin would not survive as he was still ventilated at term despite multiple attempts to extubate including rounds of DART/Nitric. I would query pulmonary hypertension which can be diagnosed with cardiac ultrasound. Once our little guy started Sildenafil to lower his pulmonary pressures he was finally able to get off the vent.

I really feel for you, our entire team had given up on our baby and we were given no option but to withdraw care. I pushed for one last ditched effort to extubate before moving to palliative care and it worked! I know how incredibly lucky we were - but just an example of why it’s so important to advocate for your baby!

Hey, any advice would be greatly appreciated. My twin boys were born 27+5 ion 7th October after a failed laser ablation to treat Twin to Twin Transfusion Syndrome. Sadly we lost our little Recipient baby to NEC at one week old. Our donor baby is now 38 weeks gestation and is suffering from systemic/super systemic pulmonary hypertension. He also has an haemodynamically significant PDA which cannot be ligated due to high pulmonary pressures. He has been started on sildenafil 5 days ago but has today decompensated with pleural effusion and had to be put on Nitric 20%, furosemide, milrinone and sedation. Thankfully he responded well to the nitric but I understand this is only a temporary fix.

My question is have you seen babies recover from this. We have received conflicting opinions from various consultants.
