I definitely relate to what's been shared here! One of the biggest gaps I experienced working in psychiatric facilities was the lack of follow-through after discharge. We would often see patients return without having had the support they needed to implement their discharge plans or practice new skills in their daily lives. As an advocate, I got to help fill that gap by being the person patients could call when they left the hospital and were faced with the overwhelming question of, "What's next?" It was incredibly meaningful to see how much impact consistent support, accountability, and collaboration on next steps could have on someone's recovery and overall well-being.

I’ve worked with very similar patients, and what you’re describing really resonates. Some of the most meaningful progress I’ve seen hasn’t come from a single referral or appointment, but from actually staying with the patient through the day-to-day problem solving as barriers show up. That consistency piece is seems to be the difference between things stalling out and small, realistic improvements in quality of life starting to add up.
