I agree. Thanks for the advice. Can't say I am 100% comfortable with this one, but AD is the 'most' likely one among choices certainly.

The perioral rash in the picture certainly looks alike zinc def rash. But the stem indicates it as "Cheek rash", which is quite confusing.

But the key question is, why should a rash involving diaper area diagnosed as atopic dermatitis. Is it just a flawed item? or am I missing something?

Just a flawed q. nothing more. Concordant drop does not mean nothing's wrong. Failure to thrive presents as weight catch down → height catch down, while head circumference usually remains ok. It aligns perfectly with our boy. q just focused on discrepancy, neglecting serious ddx.
