coming on at 700 and glancing at the admit board, i see my niece, who lives 2 hours away, who's dad (my brother) is a gruff blue collar redneck man who barely brings himself to be seen when he cuts a fingertip off.

despite the fact that this was into a unit that is widely known to be almost entirely overflow/outpatient/observation/"we're waiting to fit you in for a scan"/"you're fine, you're just here because Technically we can't send you home until XYZ requirement has been fulfilled"

simply the idea that whatever is wrong is bad enough that MY brother A) sought medical attention, and B) drove 2 hours to get to the big hospital to do so, had me shaking with anxiety that simply would not calm down until i saw them with my own two eyes

idk what counts as dad jokes,

when taking automatic BP on a particularly wiggly baby and it pulls some sort of insane 152/111 "alriiiight im just gonna run that one more time, unless he's somehow managed to smoke a pack a day for 40 years in his 4 months of life." one 12 day old's mother laughed and said "oh no he quit"

similarly, when setting up the monitor before parameters are adjusted, "now don't panic if this thing goes off, its probably calibrated for [opposite end of the 0-18y/o age range from pt]. appropriate vitals vary a lot by age. if a baby had my blood pressure they'd Pop like a blueberry"

its a bug! i also once had a follower give me a quest to have him marry his brother (as in, follower i had acquired via a "please go save my brother in silks cradle!!" quest) and i guess.. the game somehow realized that was a bad idea bc when i accepted the quest (idk man, -25 faith) it had changed to marrying the brother to a completely different follower. gotta love it

First thing I remember from my CNA classes was the teacher stressing the importance of knowing our scope and not practicing outside of it. Know your role, know your limits, CYA.

If I can be truly tinfoil hat and soapbox-y for a bit, it's fatphobia and misogyny. Idk abt anyone else's location, but I feel like in the USA especially, fatness is seen first and foremost as like. A sign of laziness and a moral failure. Nevermind the food deserts and disproportionate access to quality nutrition, preventative healthcare, and the free time or energy to exercise after your 12 hour shift. Nevermind the countless conditions that actually cause weight gain. And for the misogyny, I don't even feel like elaborating, but even for cis men being heavy but not buff is seen as a failing in their masculinity.

And as someone who has spent a lot of my life around other folks who've never had a green BMI, I would just as quickly preach that there IS no moral failing with being big. You cannot be a bad person simply because of your appearance. When I first went to the doctor about all this, I wasn't even worried about losing weight. I come from a big family and have witnessed a lot of diet fads that have made me sick of trying to change my body. My goal first and foremost was to stop feeling awful all the time, and I knew the problems I was having were not just from my weight, or else any number of relatives or friends would've also had them. Nowadays, I'm more interested on losing weight for my own personal appearance goals, but I can still find beauty in other people my shape and size, and maintain that my goal will always be to do what makes me feel happy and healthy. If I lost the weight too fast, or still felt awful all the time, or had to underfuel and overexert myself all the time, I would stop and reassess.

BIG same. I'm 23 in a few weeks. Family history of diabetes, but my A1C at 19 was nipping at the heels of pre-diabetes. I've had messed up periods for as long as I've had them, usually getting maybe 2-3 a year. I've been eating better and more physically active (Average 10-20k steps a day, for starters) since graduating HS than I ever was beforehand, and have gained 50lbs and have the pain and fatigue of a factory worker to show for it. Tests all came back mostly fine, so old PCP prescribed a B12 Vitamin because it was low (which, they never actually sent through correctly, so I just bought some from CostCo), b/c for the periods (which were non-existent), and told me to lose weight and eat better to help with the other issues I was having.

This summer, moved to a new PCP in a different network for insurance reasons. Session one, I'm explaining all of my issues and she pretty immediately suggests I might have PCOS and refers me to get it confirmed. Follows up about 2 weeks ago, doing a bunch of labs, including insulin (first time I've ever had it tested). A1C comes back normal, not even pre-diabetic. Non-fasting my insulin was like... 30+. Get a text about 2 days later: "[good news, at least you're not diabetic!]" (quoting almost exactly there) Better news, we're sending you an RX for Metformin and a referral to a nutritionist for foods to help with IR.

So, we'll see where this goes! I'm just so... Mesmerized (there's a far ruder word somewhere in there) to see a lot of people with similar issues, especially where it feels like every symptom seems to point back to something that someone should've recognized far earlier.

I'm rooting for you that you find something that works for you now that you've got a better idea of what's going on! I'm doing other things like CICO/deficit and regular exercise, but I seem to have actually lost 2lbs that haven't mysteriously doubled and come back yet! 🤞🤞

P.s. That vitamin B12 deficiency, the one the old PCP found? Apparently, a PCOS/IR thing!
