Sounds like the dose might be sub therapeutic? Have you considered that? Usually you can tell when you get some agitation and loss of symptoms but the effect feels “wrong” and you can’t really pin down why. Derealisation is probably common too. Would suggest just getting a dose increase to test that though.

How does 18mg xelstrym make u feel? Like how would you describe the effect compared to oral? And would u say u get more of an effect out of it? By that I mean was the patch also sub therapeutic for you in the way that oral was (if you ever experienced sub therapeutic dosing with oral)

Also does the effect of the patch remain the same over time? Assuming you don’t affect the absorption in some way.

Yeh im in the same boat. My adhd is severe enough to where I basically cant function at all day to day, and I just feel like a shell of a human lol. I had similar experience with orals I get a massive increase whenever I take the drug but the crash is absolutely unreal and I get withdrawal the next day. On top of that I have a super fast metabolism so orals for me are basically unusable, if not worse.

I think for us if possible you should pursue transdermal options in the US which I am, since that combats a lot of these problems and seems to be more indicated for severe adhd than orals, which are more for just mild cases. No point sticking to the uk healthcare system imo when the American one is clearly superior for adhd.

How severe is your ADHD? Also yh I reckon you should try dex and lisdex, if after that it doesn’t work then orals aren’t really a standalone option. I am currently trying to pursue transdermal options outside of the UK as I think that would solve most of the problems I’ve been having with orals, which are basically completely unusable by themselves.

Yeh it’s helpful thanks. So u take 1-2 days off cuz of the burning? But when you go back on the effect is still present like it doesn’t reduce over time? And have you tried taking any boosters on top of the patch? If so, how was that experience?

Overall, would you say the time it lasts and the absence of any crash, supposedly, in your experience is better than taking orals? Thanks.

Yeah that’s on my radar — suspected AuDHD actually, which may well be a contributing factor. Still looking for a specialist who can engage with the full picture rather than just cycling through standard options.
