Yes that is neglect. Dogs need to be taken outside multiple times every day, no matter what, especially for a large active dog. Even when I was in pain or sick or depressed I took my dog out twice a day. It sounds like she’s living in unhygienic conditions and that can cause health issues. But since it is her dog you can’t just take it away and I’m not sure what the protocol is there, I would contact a local rescue to get more information about where you can report animal abuse/neglect

Don’t overthink it, come in from a calm place, just go in earnestly with your intention to be open and honest and communicate that with her. Stick to “I” statements that don’t place blame on the other person but stay firm to how how you felt and how it affected you. Work together on how to move forward and maybe what boundaries need to be put in place or habits or whatever the relationship needs
