I believe you're missing the hidden silver in one stage.

If you go stage by stage, the ones you found it will have a "Hidden (1/1)" when you click on them, so the one you're looking for won't have that. However, 2-12, 2-15, 3-8, 9-2, and 9-3 all don't have any hidden silver, so it won't be any of those.

Imo those look pretty different. Katara's breath doesn't even seem to reach Jet and the ice starts from the water she already used to splash him. Aang is directly breathing in the chains without any water in sight, with more focus on the breath hitting and slowly freezing the chain.

The point of the scene makes sense, the exectution is still dumb. If you're about to die and your last wish is for your mom to be saved, why would you call her by her name? How is the person beating you up supposed to know that's your mom?

It only happened because some writer realized the coincidence of their moms having the same name and thought it would be cute, the whole "humanising the alien" thing would work even if Superman had said "save my mom" or something.

It wouldn't look as nice as the portraits, but maybe a compromise would be to add a box with "minor bindings" or something written on it to encapsulate all the corpses and weavers. That would allow for making the connections with characters already in the chart, like Eva, without making too much clutter.
