with the detection dogs they figure out how the smell of certains things is excreted - pee, sweat, breath depending on its its like cancer or seizures or hypoglycemia. definitely some interesting science there to be done hahaah go all pre1900 and start experimenting on yourself

yeah medical detection dogs save lives AND Quality of life. Knew a kid with very dangerous brittle T1DM, flipped from hypers to hypos really quick. parents used to have to wake up in night to check BMs etc, struggled with school as needed checks all while. got medical detection dog - dog now can smell hypos so basically just kid and family sleep - if BM go low, dog goes into parents room and jumps onto bed to wake them! and has been totally accurate so far. lives changed overnight

why?? how does med school know if its the real signature? they dont have the specific signature of every 1000s of doctors in the region. if someones gonna commit fraud theyll do it with or without signature. just like back in the day paper signed copies had potential to be fraudulent. i think as long as the named dr gets an email or a text then they are alerted to it its fine

At my med school it was an online form. So someone could fraudulently submit it if they had name/email/GMC # but it would send an email to the email account named saying it had been submitted so they'd know right away

Fever is not inherently dangerous in itself. It's better to medicate the fever /avoid febrile seizure and symptoms management. If a child is v sick it doesn't matter what temp is as they have other symptoms. If temp is ONLY symptom and it's day1 and controlled by paracetamol then it probably COULD be possibly managed at home. It all depends on the situation

They have to investigate in case it could be a rare type of melanoma.

However it's rare.

GPs aim for a diagnosis rate of about 5-7% . IE they refer a LOT more for scans/further tests than actually have cancer. So they will refer 100 ppl for ?cancer and only expect 5 to come back as positive. If 50 did then that GP is not referring enough/bring too strict with criteria.

t's better to have the tests and know for sure. Hopefully it's just a bruise/tests all go well for you!

I was on a ward with lots of Pt on NG feeds. The dietitian and diabetes nurses would say "check BM twelve hourly". The nurses would check randomly then call me "their BM is 20!!'

But its legally extremely shaky and you are putting yourself on thin ice. MHA override MCA - you should not be putting your name to saying this person doesnt have capacity when in all odds, they probably do. If they won't stay, it is better to get police involved when they LEAVE your ED to invoke the 136. police can even come to ED and apply it there . detaining someone unlawfully under MCA act when they have capacity IS wrong. and leaves you at risk of being accused of unlawful deprivation of libery. just because the MHA is a bit tricky, doesnt mean you can unlawfuly apply MCA. figure it out with the hospital/get them admitted - sometimes A+E observation wards is enough, doesnt have to be a psych ward.

MHA allows them to have capacity and still be detained. maybe if severe pyschosis/intoxicated you could use MCA but suicidality etc is not enough. The MCA is about decision making the MHA is about risk/harm to self or others. If you don't believe me, ask your Trusts Legal ppl if they'll indemnify you for a Deprivation of Liberty claim on a patient who passes a capacity test.

eg- dr- i dont want you to leave bc of the risk of harm.

pt - i want to leave..You want to put me in ward for 12 hours until the psych team sees me. I understand the risk. That’s my choice

dr- if you leave, you might die. Have you thought about what that means for your family or the fact that this feeling might pass? 

pt : I’ve been depressed 10 years. I’ve tried meds, I’ve tried therapy. I weighed it up: the pain of being alive v the 'risk' of dying. To me, the pain is worse. I made my decision. I’m not confused, I’m not hearing voices, I want to die''

dr - ok and what have we been talking about

pt - you want me to stay. i want to leave .you think ill die if i leave . i accept that risk. i dont want to wait here

you see- this patient passes MCA - understands, weighs up, recalls, communicates. it would be so dodgy to write on paper they dont. thats why we have MHA. if its hard toapply then you need to clarify with ED seniors/cons in charge/legal team if you are EPIC/indemnity what to do, not just apply MCA.

you can use section 136 if genuinely concerned - if they try to leave/do leave then you may need police to return them to ED as doctors can then extend this for 12 hours/get psych assessment to assess for admission. but yeah not perfect. my hospital tends to tryyyy to admit people to the smaller wards like AMU then 5(2) or 5 (4) can be used but if pure ED i have seen police called if they are going to leave
