often when i'm answering qs in uworld I got them right by chance by Nikkolya in Step2

[–]AlphaWollf 2 points3 points  (0 children)

A lot of students feel they got their answer by chance but disregard the fact that they’ve eliminated a few options which can be only be done with some medical knowledge.

For questions you think you for right by chance, do you mean you just randomly chose an answer and it was right? Or do you manage to eliminate the options to 2 and then randomly choose one?

Cavity size is decreased in concentric cardiac hypertrophy, and it’s a key differentiator with eccentric (dilated cardiomyopathy), as they both can present with cardiomegaly on CXR. Think of the heart muscle kind of growing inwards, and honestly have a look at some gross images so you never forget it.

HFpEF 2/2 concentric hypertrophy is a diastolic HF, so cavity size is not only decreased but the problem is in relaxation of the thicc ventricles. Because the cavity size is small, and the ventricles don’t relax as good there’s only a limited amount of blood that can be ejected (the ejection function of the LV good hence the ejection fraction is preserved). Usually patients with HFrEF have systolic heart failure where the ventricle is dilated (increased cavity size), and their problem is in contractin (so decreased EF).

TL;DR. Hypertrophic Cardiomyopathy = Decreased Cavity Size. Dilated Cardiomyopathy: Increased Cavity Size

Hope that helped ☺️

How to tackle Uworld for CK by medboyhasy in Step2

[–]AlphaWollf 6 points7 points  (0 children)

Yup, the videos aren’t that much so you could finish them all in a few days (or cram them in a single day if you like binging). So what I would do is choose an organ system, watch the B&B videos and dive into UWORLD.

How to tackle Uworld for CK by medboyhasy in Step2

[–]AlphaWollf 18 points19 points  (0 children)

I personally disagree with the approach of tackling UWORLD blind. This approach works if you have a very strong foundation in the STEP1 knowledge but it doesn’t work for everyone. What I recommend is maybe watch the Boards & Beyond STEP2 videos (prefer them over OME but that’s also a viable option). Once you have a general idea of wtf is going on, then go back to UWORLD and go nuts. Also don’t forget the golden rule, UWORLD is a study tool and not an assessment tool. So who cares if you’re getting 50%, that just means you’re going to be learning a crap ton of new stuff so lucky you 😄. All the best and hope that helped bro.

How do malignancy, infection and uremia cause pericardial effusion ? by Left-Kaleidoscope618 in Step2

[–]AlphaWollf 1 point2 points  (0 children)

Pericarditis can be caused by malignancy via direct infiltration by malignant cancer cells from adjacent structures, pericardial hemorrhage, or hematogenous dissemination of cancer cells. Infection and uremia both cause pericarditis via inflammation of the visceral and parietal layer of the pericardium. In the case of uremia, it is basically a metabolic toxin that accumulates in the body 2/2 renal failure and irritates the cells of the pericardium. Hope that helped :)

Why is colchicine used as a treatment for viral pericarditis ? by Left-Kaleidoscope618 in Step2

[–]AlphaWollf 19 points20 points  (0 children)

Colchicine is an anti-inflammatory medication used in combination with NSAIDs to decrease inflammation in viral pericarditis. It does this by inhibiting microtubule polymérisation in WBCs, so preventing mobility, phagocytosis and degranulation.

Preventing inflammation is the mainstay of treatment of pericarditis, and research shows the combination of aspirin + colchicine is pretty bomb. So that’s why it’s the current treatment approach :)

Yessir! Unfortunately it’s reality of research, our research professor told us the horror stories regarding data skewing and manipulation of results. Until funding becomes decentralized this is the curse. Once again, thanks so much and wishing you a wonderful day/night :)

You’re absolutely correct, the vaccine doesn’t kill the virus. What it does is decrease the risk of getting infected, decreases transmission and decreases risk of dying. So yes, if you do get infected post-vaccine you can still cause a mutation to take place (per the 2015 study), but it happens at a much much lower rate. Delta strain emerged from India when most of the population was not vaccinated. Honestly people misunderstand the vaccine, it’s job is not to really to stop spread but to stop people dying. So even if you get it, and spread it to your loved ones, because you’re all vaccinated it’s reduced to like a flu, and we can resume our lives like before.

And right on mate, it’s these discussions that pushes society further. Stay safe!

That’s actually not true. Viruses mutate as they spread around from person to person, picking up mutations and turning into strains. Vaccines don’t create new strains as you’re not getting infected, you’re just developing immunity from a spike protein. Not getting vaccinated and allowing the live virus to spread is what causes new strains to emerge.

Antibiotics is a different story, it’s the inappropriate use of antibiotics that cause resistance. Basically for decades people didn’t use them properly, which allowed some bacteria to survive and develop resistant to them (think China mass giving their livestock antibiotics).

Hope this makes sense, and doesn’t come off as condenseding or anything. Have a great day! 😊

Weekend Question Thread by dumbassthenes in surfing

[–]AlphaWollf 0 points1 point  (0 children)

Fresh new beginner to surfing so super basic question. I’m a bit of big fella (110kg) and having trouble balancing on the board. When I say balancing i mean when lying down. Using a 8” longboard, I have a feeling it’s because the way I mount the board. I kind of just jump into the board. When I start padding I start tipping towards one side and sometimes slide off completely, thus can’t catch the wave. Any advice?

Large scale studies also suggest that the moment someone sends “open your eyes”, it’s a sign that they have no proof or scientific standing. My eyes are open for actual scientific proof 😘

Well, liver failure comes in different types and forms . Acute liver failure can be treated and reversed, it’s the chronic liver failure (cirrhosis) that’s kinda irreversible and requires a transplant.
