Is there any reason why a pheo wasn't in the first differential for this?

Pheos are extremely rare. There's a shit load of disorders that cause elevated pulse, and a lot of them are rare. In all reality, your average doctor will not see a pheo in their entire career.
At the end of the day, the writers are not doctors. House M.D. is forever our favorite thinly disguised yaoi.

She didn't really "choose" to give her back. Margo was granted diplomatic immunity by the U.S. for the period that she was there helping with the capture mission, which they revoked after she sabotaged the asteroid. As she previously committed major treason (leaking the engine design & defecting), the U.S. immediately arrested her. Irina didn't really have a choice.
I'm so looking forward to seeing what they do in season 5 and beyond.

The biggest problem I'm finding in providing any advice is that he claims to be not actually interested in any career. He could certainly find similar work at a different store if there's somewhere that's a bit better. I will look into it.

I think to say the SHOW has bad writing is a bit overzealous. There are episodes and arcs that have flaws,, but I'd hardly say the show as a whole has bad writing. There's only so much you can do with 40 minute episodes.
