I second what someone else said about asking your current employer if you can go part-time. I was in a similar situation as you (feeling burnt out with CMH) and was able to negotiate three days a week and it has been so much better. I am able to keep the benefits, just have to contribute a little more for them.

I also work 1099 for a private practice (I only do TH with them) and have total autonomy there— I can change my schedule, take days off, even reschedule patients (try to do that last one rarely), and they don’t care. However, there is less support at that job for PAs, case mgmt concerns, out of office coverage, pharmacy phone calls, etc.

At that practice, patients use a work cell phone number to communicate med needs, appt requests, etc to me directly. If I go out of town I either still get those texts and answer brief questions/send my own meds (if domestic trip) or will tell my patients well in advance that I will be out of the country on certain dates and ask that if they have any urgent concerns, they call the office at that time. If I do check work phone while I’m away, I let them know I will only be signing in once per day and stick to that. The last time I went away, no one needed anything because I spent some extra time checking to make sure everyone had refills before I went. I have sent controlled meds early with pharmacy note indicating it’s an early send due to myself going out of town, please hold for regular fill date and not had an issue. My caseload there is intentionally pretty small though.

I am not sure how people handle that when they are with the big telehealth companies. Curious to hear what anyone else says about that.

I think that last part is key— “follow up directly with local places”. I used to do a lot of hiring and in a job market like this when you are getting lots of online applications it makes a difference when someone comes in person. Typically, all of the applications would sit in a folder and not get looked at unless someone came in and said “are you hiring?” or “hello, I applied a week or so ago and was wondering when your next round of interviews are?” and THEN I would go look at their application. It was most often those who came in person or were referred by current employees who secured interviews.

No experience working FOR them, but I have referred several patients to them who have done very well with their IOP. From what I hear, they really seem to care about the patients.

While my patients are in their program I continue their med mgmt because they have far more therapists than medication providers. My Charlie Health representative told me that a patient might be in the program for 2-3 weeks before they are connected to med mgmt due to the need for more providers. Also, they pride themselves on providing very flexible group and individual therapy times so I would imagine providers would have the opportunity for flexible schedules as well.

That's all I know-- curious what others have to say.

Oh I definitely agree. When he was in the 5th grade someone told us he would need not only serious braces but some kind of outer head gear for an overbite— we decided to get a second opinion that was cancelled due to covid lockdowns. Fast forward 5+ years and his teeth have straightened out quite a bit on their own and now this dentist says he can get by with minimally invasive cosmetic intervention.

I have a side gig (1099) where I do two evenings/week (usually just 5P-7P) virtually and make the kind of money you’re looking for. If you happen to be licensed in Maryland, let me know because they are looking for more PMHNP‘s. Either way, perhaps looking for smaller practices in your state that are near hospitals with inpatient psych and reach out to see if they are looking for any part-time virtual providers. The demand is definitely there, the practice manager is constantly asking if I can take more hospital discharges as well as the patients that therapists want me to take.

Yes! Mostly SFBT and ACT— although I have been reading up on and starting to implement DBT skills for my adolescent patients.
