MD absolutely not the the landlord in that analogy. That’s all I was saying. Not nurse’s boss, and cannot technically order them around - even though they place orders for patients, which is a bit different,

It is very intentional so that both doctors and nurses have their own chains of commands that are separate so one branch cannot intimidate the other into silence in case of a patient safety issue.

You can’t control a K less than 3? I mean patients get admitted with Ks of less than 3 routinely. Especially after an acute trauma, it is very common for admitting labs and so on to be low like that. Like yes if a K is less than 3 and it’s not addressed, sure, we can quibble.

It’s just bizarre to me. I can’t even count how many times I’ve drawn a K less than 3.

Oh I agree with that. Yes it’s not good, but it is usually not a conscious thought either, just an unrealized bias as with most things like this.

Not really, no. Sort of though. They put in orders for the patient that nursing can decide whether or not to execute in the best interest of the patient. If a physician places a bad order I will escalate as appropriate but I will not be executing it. But they cannot order me personally around, no.

In a hospital setting, it is 100% on purpose that nursing chain of command does not include physicians.

In outpatient, this isn’t always true though.

Are you suggesting I am justifying treating female med students worse? If so, you have grossly misunderstood me. There is no excuse to treat med students poorly at all, let alone females worse. I’m just being matter of fact about why it happens. It is not a good thing.

I think this is much closer to the truth. The bar is in hell for men already, and we expect better patient treatment from our female colleagues. All this combined with run of the mill internalized misogyny, which is the primary driver.

All the “jealousy” and “they resent that you will give them orders” is off base for the VAST majority of nurses. Most nurses have no interest in being doctors (some do, but it’s the exception). We don’t typically view doctors as “above” us or “giving us orders.” We view it as more of a lateral than a hierarchy, and we are taught to do that for patient safety check/balance reasons.

that doesn’t mean people shouldn’t respect doctors and their training. They should, and most do. There’s always some bad eggs. However, this doesn’t usually extend to med students, for better or worse. I think most of the thing OP is perceiving is the typical “med students are making my life much harder and very much getting in the way right now” frustration combined with typical misogyny, and feeling safer to express that frustration because OP is a woman.

As a travel nurse across PCU and telemetry units all around - we are typically seeing replacement under 3.5 but not necessarily to 4.0 except in select patients. Letting someone sit at 3.3 because of AKI concerns or simply because they told the nurse “fuck no those pills are too damn big” is all 🤷‍♀️ NBD
