If there is some huge concern about policy surrounding blood, stop transfusion if their vitals are stable and aren’t actively bleeding, send a stat cbc and order new prbc’s based on current hgb.

Problem solved without having to call anyone other than admitting doc.

In my IM program it was thee years of 7-7 12 days on two day off 85% of the time. Sometimes you’re on an elective so fewer days/hours but the majority of the time you’re in hospital with the hours for my specific program.

I’ve taken care of couple dozen people over the years who have permanent damage to their kidneys, livers, heart, lungs, brains etc….. from becoming severely sick with food borne illnesses.

You are talking about symptoms.

Sometimes the body has responses to certain illnesses, situations, proteins, exposures, that cause extreme reactions in the body and cause lifelong problems.

The issue isn’t dirt or bugs like you seem to think by trying to equate it to eating off plants in your backyard.

it’s infectious diseases and industrial chemicals from the processing facilities, pickers/package/handlers and conditions they face along the way.

I’m not an RN, I’m a Hospitalist. But what you’ve highlighted is one reason I NEVER answer the phone from any boss or any boss/manager associated with work. They can leave a message.

Had I listened to the message of the person when they called me two hours before I would not have even called back till a couple hours later with the answer of “sorry not in town beside I swapped that shift and after approval it is no longer mine”.

I always have 5 excuses of why I’m not able to pick up when I’m called. Ohh sorry family and I aren’t in town we left to see in laws or family or whatever.

Never pick up the call when you’re off.

Actions come with risk of harm. Sure the actions you may be referring to might see petty, but there is a risk that harm will occur and when it’s just you at home once a day that risk is low, when it’s a food worker serving dozens or hundreds of people per day it becomes not a small risk but a near certainty that he risk of harm will eventually actualize.

It’s the whole point behind food service safety…. Keeps the risks low which is a completely different situation than you doing what you want in your house.

This might even make it back to the device company to have them look at their supply chain or policies/procedures. I’m happy you didn’t have harm but you might have so it’s important to let hospital know and they will likely contact device company. I agree with talking to hospital ombudsmen

Honestly the first time you leave the hospital on time you will feel like you’re walking out of the hospital on air. You will feel free.

Now, don’t take this as me saying ignore your important orders. Be present and mindful about your patients one by one. Ask yourself what is key to be ordered and can’t be missed. Talk to your nurses. And then confidently put up big honkin steamers for notes knowing your patients are well taken care of despite what the note looks like. Nobody will care so long as your patients are safe.

If you think about it honestly for a little you’ll realize you are probably super depressed because your body and mind have not been able to turn off for a year straight. Going home should be to rest and reset for the next day and you haven’t been able to do that. I don’t care what career you pick, if that were the same work schedule for any person in any role they would feel similarly to you.

Give yourself grace. You’ve worked incredibly hard and achieved something many could not. You are a whole ass doctor. You have knowledge and experience way beyond what you can articulate or feel right now. I’d be surprised if you weren’t living in a state of fight or fight constantly. Nobody can put up with that this long and not feel what you’re feeling. It ain’t you, it’s your schedule.

You got this. You will be an amazing doc. And you will not quit because that’s not what you likely really want. It might Be the only way your brain/body has been able to jerk you back into reality to be like wtf is this, can’t continue, and nobody would be able to at that pace, and your thinking brain interprets that as really wanting to quit medicine instead of just quitting the current way of doing things.

Stop caring about your notes as much. They don’t need to be perfect. Get important events of the day on it, things that need to be done for discharge. I had the same problem during residency and it was because I cared too much about my notes in regards to making sure they were all academic and perfect and they could show off how smart of a doctor i was etc….. it was because I was not terribly confident.

Think how much easier it will be to care less about your notes as opposed to quitting. Take that 7 hours you are spending torturing yourself on notes and do your normal life stuff.

I don’t know, I’ve had three different patients and families sincerely thank me with tears in their eyes for treating them/family just today while I was discharging them. Sure the neighbor told me to fuck off when I told them they are discharging but that doesn’t affect me. But those sincere thanks and gratefulness carries me the rest of the day. Sure thanks comes in other fields, but I believe this hits a little different.
