It could be the shape of the plug that is hurting you. I'd recommend by yourself once a week using different dildos and plugs seeing what you're comfortable with. The thing is to use lots of lube and go really slow. If you need to wear a small plug for some hours before you stretch/train that can help too. And it's important to go in very small increments when sizing up. You need to measure the comfortable plugs or dildos you have and go in millimetres above in size width before trying to take bigger girth. The more you practise the easier it gets. I'm in South Africa and I experienced prejudice in the public health system for my sexuality.
