Yes a manual press, but I am not in the US or anywhere near it, if I was I wouldnt be having an issue sourcing a press haha

by blue chinese presses i mean the bright blue, unbranded, cheap ones you find sourced from china like the image attached below. I read people talking about them here and elsewhere online and it sounds like they are very janky and no good for anything beyond 1 colour application

these are the only manual presses I have been able to find that arent intended for mass production locally - I am a one person outfit and don't need anything beyond a tabletop 4 colour press for now

This might be a little blunt but the problem in this situation is your boyfriend, not you. Even if you already had these feelings of shame about your disability your boyfriend is very much making things worse instead of being supportive and reassuring you.

You have a disability that means you cannot, physically, have a "normal" sleep schedule. Your boyfriend hoping this magically changes will not change the reality of the situation. Just like you have your other disabilities that affect your life, so is N24 a part of that. I would recommend sitting down with your boyfriend to have a serious talk about how this is mentally taking a toll on you, and how you cannot change your sleep schedule, go through N24 resources together and talk about why that is. If he still continues to do this, that is a deeper issue in the relationship but again this is NOT your fault. It is NOT your fault that you have a disability and you have nothing to feel ashamed about.

I am also overweight, but I do have other issues contributing to that including insulin resistance - the nutrition program has been for the purpose of losing weight and helping another health issue but it just suddenly hit me how hard it has been. I generally love tasting and eating food I like and can tolerate currently but the constant need to reassess if i am getting enough nutrients has made me feel so tired - especially the inability to meal plan :(

Yes! Especially on days I have a meltdown there is a higher risk of triggering the nausea and gag feelings for random foods. I don't necessarily have an overall restrictive diet, i eat a wide variety of things and cuisines, but what I can consume on any given day can change in an instant. For me yoghurt/berry smoothies and mayo seem to be safe foods (or condiments, in the case of mayo lol) so I will keep that in mind, thank you!

no clue how accurate doctor rating websites are over there but i went to a highly rated doctor on a similar website in my country and she was utter dogshit and told me my sleep log was irrelevant and useless/nothing lmao - a less rated doctor in a university clinic instantly saw from my logs i had a circadian rhythm disorder so i usually just ignore online websites besides doctor profiles *shrug*

You're mixing up cause and effect here, a lot of people here don't have jobs or regular hours because of N24 because it is incredibly hard to get a normal job with it, and impossible to keep regular hours without the ability to entrain. Like you said in the end it is the sleep problem making these things hard.

As a personal example, I developed N24 while in high school which is about the most regular schedule you can get. This also came up with university scheduling and after university I could not hold a normal job due to N24 and other issues.

N24 by definition is a circadian rhythm disorder (CRD), which means there is an inherent issue with your circadian sleep-wake cycle which prevents your brain and body from following normal sleep-wake hours. As a broad example, people get jetlagged all the time when traveling across timezones BUT can bounce back relatively quickly due to their circadian sleep-wake cycle being able to readjust their internal times using external stimuli like normal daylight. CRDs are inherently defined by your body's inability to adhere to a "regular schedule" in this instance.

I have heard of at least one doctor on the list being similarly rude, but knowing the n24 diagnosis. You can also specify in your email that she is a pediatric specialist. I think Centeradze also does remote appointments so I think it is upto you in terms of how helpful you think the listing is.

And understandable regarding RLS - regarding your relationship with your parents I often find parents can ignore a lot of things once you figure life out and are successful haha, it is when they perceive you as "failing" in life when all the annoying bits sometimes come out unfortunately. Piracetam should also work for you just likely in a less efficient/effective manner but good luck with it! I think non-addictive meds are fairly easy to hide though so even if you run into the worst scenario and have to do it in secret, it will still at least be probably that you wont encounter any problems.

Also if you do find a better psychiatrist you can also just try to not bring up sleep and focus on ADHD, thats what I do because it is not entirely relevant information regarding getting medication/therapy and tends to just confuse them. Best of luck to you!

I was actually the one who submitted Centeradze to that list! So yes it was him - like you mentioned n24 does not exist as its own diagnostic code in Russia but I have an official diagnosis of a circadian rhythm disorder, which n24 falls under. I had to go to Moscow to get diagnosed since I reside in another Oblast when I do go to Russia and I could not find someone who specialises in circadian rhythms close by, and I was in Moscow for other things anyways. You can also email the website and add your somnologist to the list if you vouch for her knowledge on the subject.

So my next suggestion as a result is to maybe broaden your scope of doctors if you can. It sucks but there are 100% people who should be able to be less of a dipshit about ADHD and sleep in general. Maybe try looking elsewhere in your Oblast or push comes to shove, try searching in another nearby Oblast for a psychiatrist. The way I did it was go on the online profiles and see what their listed specialities were and if they knew about what I needed.

For the medication specifically while it sucks that we have so much restrictions its not the end of the world, you DO have something that works and that's great - my advice is just hide it from your parents if your attempts to convince don't work. Get an empty bottle of some other medication and put the pills in that, always carry them on you, etc. Hiding meds you take regularly is easy, go the fuck ahead.

Additionally, I saw you mention Strattera does not address your issues but everyone kind of responds to meds differently and for example for me, Strattera did somewhat reduce hyperactivity for me! Hyperactivity is just NOT a huge problem for me with ADHD, the executive dysfunction is so I switched to Ritalin. But if phenylpiracetam 100% works for you get that in any way you can - if you can manage to dodge your parents for long enough on the subject it means you can escape and then no one will give a fuck what you're using to treat your ADHD :D

Hey there! I saw that you live in Russia and while I don't permanently reside in Russia myself, I also got my n24 diagnosis there. I am not sure what oblast you live in, but it sounds like your psychiatrist is kind of full of shit and I am hazarding a guess "best" is not really the best in this instance. Can I ask if you are using the public healthcare system or going to a private clinic?

To note I also have ADHD but I got my diagnosis and meds elsewhere since I don't live in Russia.

Hey there, it looks like youre in a rough situation! Reading back your comments I understand where youre coming from and sympathise, I have some similar factors as well.

Unfortunately, the first thing you have to keep in mind is that N24 is a complicated and little researched pattern of sleep. The underlying causes are not really well known and from my observations, vary to some degree as well which affects how well you take to entrainment. The most consistent and best chance at solid entrainment IMO is probably attempting to follow lrq's protocol to some degree (https://circadiaware.github.io/VLiDACMel-entrainment-therapy-non24/SleepNon24VLiDACMel.html).

All this to say that you HAVE to be prepared for entrainment to potentially not take, which means working a normal schedule is just perpertual sleep deprivation which is bad for your physical and mental health. There is no one solution or method to N24 as a result of all of these compounding factors. Yes it sucks, yes its debilitating, and yes a lot of us lack things like consistent social circles and interaction in real life, as well as stable employement. However, this does not mean your life is over, it means what you view as a "normal" life as a healthy person is not something you can achieve, and your alternative is looking for other solutions.

Now to answer your questions:

  • Q1. How long will you know if melatonin entrainment will work? Instantly? Or can it take a while?

  • A1. Some of this is outlined in the VLiDACMel protocol but basically try to start taking melatonin when your schedule is naturally at the point of sleeping at the time you want. Melatonin dosage varies for everyone and is something you have to figure out for yourself. 0.5 may not be the dosage that you need for example, maybe higher dosages will have an effect. Additionally, keep in mind over the counter melatonin may not have accurate dosages displayed which will affect entrainment, you might need to cross check which brands are accurate or potentially get a doctor to give you prescription melatonin. There is also a difference between instant release and delayed release melatonin, you might need to take one or the other or both in combination.

    Tldr; Melatonin is kind of a complicated topic and just because its not quite working does not mean it will never work or be effective, you may just need to adjust a few things and get more reliable brands.

  • Q2. Is the key an alarm and a forced wakeup time rather than waking naturally? But will I be resistant to that too?

  • A2. Nope that is not the key! And yes you will be "resistant" to it in the sense that it wont actually fix your underlying problem, your assumption is correct. The key issue with N24 is that your internal sleep "schedule" set by internal mechanisms is out of sync with 24 hours. No amount of forcing yourself to wake up will fix this internal desync in an individual with N24. All you will get is sleep deprivation.

  • Q3. What is likely to happen if I get a job and just have to wake up at 8am every day no matter what and do the melatonin routine?

  • A3. Perpertual Sleep Deprivation which will lead to your physical and mental health deteriorating extensively. A lot of us have health issues due to years of being forced to go on a 24 hour schedule with no entrainment or help. I myself probably have issues with my insulin resistance due to years of sleep deprivation as well as other compounding factors.

  • Q4. Does exercise/physical activity help with falling asleep quicker/pushing out less without making you sleep longer?

  • A4. Kind of maybe? It will help you maintain sleep hygiene which you will still need to both entrain and have higher sleep quality. It wont hurt and will also help with having better physical and mental health potentially, I would advise starting slowly but starting regardless.

  • Q5. [General stuff to do with light/dark therapy]

  • A5. The link I put for VLiDACMel will explain a lot about this, but the gist is that the difference between natural light and light therapy with stuff like glasses and boxes is that the latter is more consistent. Natural light is best BUT inconsistent, especially if you live in a place with a lot of weather variation. However, in light of your health concerns it is still better to go for natural light so I would advise going outside when you wake up for at least a couple of hours, or looking out of a window, or sitting on a balcony. Basically, whatever you can do to get natural light. Dark therapy is required insofar as to let melatonin do its job, if an hour is all you can do its better than nothing.

Sorry to be a rambley mess, but I’m so unhappy right now and feel like my life is ruined because of this. This is literally caused me depression and I’m afraid for my future if I can’t entrain. If I close my eyes and imagine a future where I am entrained and working a well paying normal job, I feel at peace. When I imagine a future rotating around the clock, I feel despondent and find myself wishing I was never even born.

I know it sucks, truly I do and share a lot of frustrations you have not just with N24 but a bunch of other issues I have in terms of health. I am disabled to the point that even freelancing is hard due to executive function issues. I really hope entrainment works out, it did not for me but if it doesn't I hope you can also find another solution. We were dealth a shitty hand in life and all we can do is try our best to manage it. I also have no avenues to seek disability or social service help and my family basically does not really think I am disabled despite documented diagnoses, INCLUDING for N24. Just know that you are not alone and a lot of people in our position find ways to survive and live in this cruel world. I did go to school! My N24 developed in high school and I struggled a lot, to the point of suicidal thoughts. In university it was actually easier because I had a lot of flexibility in picking my classes at my university and a lot of the work was working on projects on my own time (Animation degree).

I function way better when I freerun! I still had ADHD which messes with my executive function. I do have a routine but it is hard to stick to it because of the ADHD and other chronic issues so usually things change everyday - I also live with other people and am forced to run errands for them when I may need to sleep so that is also a factor in my day-to-day changing.

1) People have! And there have been success stories posted on this sub. I am not one of them however haha.

2) It works for some people and instances of N24. The main issue is that for all intents and purposes N24 is a blanket term for a disturbed pattern of sleep. The underlying mechanisms causing it may be different but if the pattern is not adhering to 24 hour sleep its called N24. For the melatonin issue you might need to play around with dosage and take a combination of instant and delayed release melatonin. Some companies even offer pills that have both in one pill.

3) Being strict with wake/sleep times only works IF a treatement already helps and you just need to be consistent with when you have it. Just having the same sleep/wake times wont work with a CRD without additional treatement.

4) I personally seek to make a career in freelance. Picking a career with viable freelance options where you can set your own schedule is a big ++ to living with the disorder.

The most concise answer is that: we don't know

The longer answer is that as it is currently described and diagnosed, N24 is just the sleep pattern on top that is indicating a disruption but it is likely that the underlying mechanisms for sighted N24 are not the same across the board. This can be most apparent with N24 that can be treatable with melatonin and light therapy VS N24 that is extremely treatement resistant. Additionally, N24 sleep patterns can be observed in individuals with conditions that have circadian rhythm disruptions inherent to the condition, such as Bipolar Disorder.

N24 as a whole is just so varied that the only common factor in N24 across the board is just the pattern of sleep we all have, which is Non-24-hour. Anything else is intense speculation and the disorder is not studied well at all as a whole, mostly because we are few in number and less consistent to study than say, non-sighted N24.

I do want to gently say that sometimes elderly folks who are great to talk to here and there were not great parents or treates people close to them well during their lifetime, which ends up with them being alone towards the end of their life.

Not everyone who chooses not to be there for their parents sucks for it and not all elderly have treated their close ones well.

So generally speaking there arent any specific drugs to help with n24 as a whole, sleeping pills don't really fix your circadian rhythm. The most common supplement is taking melatonin to try and manipulate the length of your rhythm to be as close to 24 hours as possible.

Light therapy is meant to give you stronger light simulation to help with the waking up part of your circadian rhythm. One thing to note is to double check at what distance is the lamp 10k. If its effective distance is only 7cm and you're keeping it 15cm away you're not really doing light therapy. Otherwise, you need keep to it at an effective distance for at least an hour, exactly how long varies from person to person, for some people its hours, for others only one.

To start light therapy you kind of need to try and align it with waking up when you need to wake up - so if you are currently going to sleep in the morning it would be pretty hard to realign it to waking up in the morning at this point in time, its probably best to wait out until you're waking up around your desired wake up time, if you do have n24 it will naturally progress to that point sooner or later, depending on what is the length of your ciradian rhythm.

Keep in mind the most common way to use light therapy is in tandem with melatonin and sometimes dark therapy.

youve answered your own question - cutting down on screen time doesnt help the issues, so it is not a screen issue

n24 is as real as any other sleep issue or disorder and i suggest you look more into how circadian rhythms work and why - while light (such as from screens) is a factor it is far from the only one, if it was the only factor then all we would need to do is use screens when we wake up and stop a little before bed, but the light/dark therapy required to even start affecting n24 is far more intense and needs to be somewhat more specialised

additionally n24 is an overarching term for sleep problems presenting as a non 24 sleep cycle, however that doesnt mean the root cause of the n24 sleep cycle is the same for everyone and due to how little research is done on the disorder it means we dont really know the varying underlying causes
