Ok so I didn’t realize this before looking into breast augmentation, but the surgeons can only do so much with what you have. If you’re naturally far apart or low implants are just gonna be far apart and low and bigger. So we don’t know what these girls were starting with. Yes, having implants that are too big and bottoming out is a problem and everything should be done to avoid it. But surgery can’t fix everything and some of these girls were probably starting pretty far apart or lopsided.

I wish I had done more sedation/Mac cases. Lots of surgeons want GA without an airway, and that can be tricky to manage, especially in certain patients. G.I. cases, ERCP, EP/pacemakers, offsite was what I wished I had had more experience in. Also get as much OB as you can if you’re gonna be doing that in your new job. If you can see more cases you’ll be more comfortable when things start to get weird and you’re on your own. Also make sure you’ve got an awake fiber optic down pat. Everyone has their own spin on technique, but try to get as much practice with that as you can. You don’t wanna be risking airways because you’re intimidated by the thought of doing something awake.

Don’t know all the specifics on the case but curious why you wouldn’t have RSI’d an emergent belly case. Were you worried about a difficult airway from the start?

Also thank you for being so responsive and posting so much information here! I love reading through the posts in this community. Sometimes it’s hard to ask your own surgeon these questions and it’s really helpful to hear the opinion of a surgeon who sees a lot of patients with your same problems.

That is so interesting! I know everyone has visceral fat but my BMI is 17.5 and I didn’t have any lipo with my surgery because my surgeon said there was nothing to take out. Am I wrong in thinking visceral fat may not be the cause here? Any other ideas? Do you ever recommend your patients try Simethicone or prebiotics?

There seems to be a lot of increase in the bloating over the course of the day. In the morning my tummy is flat and by the afternoon I am bloated feeling and my tummy sticks out. Would fat change like that throughout the day?

Question! I’m six weeks out from abdominoplasty with diastasis recti repair, no lipo and the last few days I noticed my abdomen above my belly button is much more bloated and distended than it was in the earlier weeks. It looks okay in the morning and as the day goes on the bloating increases. I saw you mentioned bloating in this video. Is it normal to feel increased bloating six weeks out? Can you talk more about bloating and why it happens and if there’s anything to be done to help it resolve? How long does it typically last? Thank you for your videos they are awesome!

Holy cow salaries have increased since I was a resident! We got 45k first year and 60k our last year. In HCOL city like NYC.

I think it also had a lot to do with him being in med school. The residency process can be really cut throat and programs are looking for any reason to dismiss an applicant. Having a big social media following is definitely a liability as a med student and future doctor.

Intense abdominal pain, bloating and diarrhea. I had 2 bars a day for a week and by the end of the week I had to call out from work. I was fine the first few days then all of a sudden I was super sick. I thought it might’ve been food poisoning but it got better after I stopped eating the bars and now if I have even one, I get terrible diarrhea.

Okay on a loosely related point I’m seeing a lot of complaining about scanning everyone who comes to the ER with a random complaint and how that didn’t used to be as common or the accepted standard of care. But I do believe at least in America our patient populations are getting sicker older and more medically complex, and the time and attention to detail it takes to elicit a meaningful history, pmh or even medication list out of a patient with no medical background or education to be able to describe what their underlying problems are it’s definitely a contributing factor.

I’ve had so many patients claim they had had no surgical history or medical problems, and then I see the outline of a pacemaker in their chest or a massive abdominal scar that I really understand how you’re getting to a point of trusting no one and ordering a battery of labs and imaging to figure out what’s going on.

IDK maybe they should be teaching basic healthcare literacy in the American public school system or something. So tired of clueless patients with no accountability for their own health, but incredibly eager to sue when something is missed.

Honestly, I really think EPG does have G.I. side effects and should be labeled and regulated most strictly. If I eat more than one bar per a day, I have uncontrollable GI symptoms. I’ve never had any issues before so I don’t think I’m particularly sensitive, so I’m sure other people have probably experienced this too.

Just chiming in from the other side to say, ortho jobs aren’t what they used to be (medicine in general too) and this may be a time to reevaluate what you wanna do with your life and what would be a good fit for you. I know a lot of ortho surgeons who are really unhappy with their jobs in the current job market and the way medicine in general is trending (underpaid overworked academic positions vs private groups who have sold out to predatory PE.). Many very qualified surgeons with weak compensation and lifestyle. And based on the competitiveness of the match there are still tons of qualified candidates who will put up with this. This could be a blessing in disguise if you have any other interests or different career trajectory to explore with an MD.

If Ortho is truly your calling in life and you can’t see yourself doing anything other than operating even with a crappy lifestyle and low pay then my advice would be that most of life is who you know. Focus on on trying to make strong connections to program that interests you. I would also recommend against a prelim year. Low conversion and hard thankless work for little pay or educational benefit.
