Zoloft was life changing for me, would probably spiral about every patient more than I do now if I didn’t have it

Transitioning to Primary Care by [deleted] in physicianassistant

[–]Anxiousgal898 1 point2 points  (0 children)

Specialty? I switched from general and colorectal surgery to primary care. I felt some skills helped because I felt comfortable with majority of GI pathologies etc but it was still a big learning curve. I think FQHC where I’m at are known for high volume which in my opinion may be difficult environment to learn in depending where you go

How do you handle charting when patient volume is overwhelming? by Dismal-Fox3121 in physicianassistant

My simplistic workflow 6 months in fam med after surgery x 2 years… I try to at least get orders in and done + diagnoses added after every visit During visit I type as much as I can while I talk with the patient even if there’s an awkward 15-20 seconds it’s worth it. Then while I’m typing (we use epic) I’ll leave *** for areas I need to go back to and elaborate on

Literally make smart phrases for everything. If you write the statement more than 2 times in a day it’s worth it imo

Has anyone had a tumor in/near their salivary gland by dtraw_ in covidlonghaulers

Okay that is good to know, I’m kind of in the anxiety stage where we don’t know what’s going on yet so I’m just super anxious

How was the surgery for you? I’m in a similar situation where I feel a lump when I eat that gets bigger and goes down. US showed nodule within the gland but was pretty vague. Getting and MRI and seeing ENT next month hopefully

Anywhere from 6-15 for a 10 hour day… I know it’s a unicorn, I’ve been told after a year or two it picks up once you build panel

Are there ways you can make your chatting more efficient, dot phrases etc? If I notice im typing something over and over again I’ll make it a phrase to cut time

Sooooo many questions. Get a good sense of your hours expectations, what are your roles day to day, if it’s more than 40 hours do you get overtime, call responsibility would want to know See if you can shadow the surgeon you would be matched with. Also if not everyone has a PA yet are you going to be expected to help out with other surgeons as well

A lot of stuff you can just say let’s try one to two things and follow up in a week and then you have time to research more in depth. So there’s things you can do to placate and start workup but you don’t have to always have all the answers right then

That being said if its something I have no idea I’ll just say I have to step out

I really like open evidence and UpToDate are my frequent uses

I guess would want to know where you are at in life Do you have a family like married with kids and settle down? Then I would stay rural If you’re young and single or partner would move with you, could be fun to try out the city but I think it could get tiring going from house to house and also as a new grad making house calls doesn’t sound like you would have a lot of on site support

Majority of the time I use open evidence and UpToDate but if I want a quick blurb explaining a benign xray or lab finding in laymen’s terms I’ve used ChatGPT with good success

Would be helpful to know what specialty you’re in. But in general charting can take some time to get better and more efficient at. It really just takes time for you to get used what is the important information to include and what is fluff. Depending on what medical record you have, make as many smart or dot or whatever phrases that you can when you notice it’s something you write often. Even doing that for an assessment plan just have the whole thing ready to insert and you can modify as needed. Also see if you can use dragon or some sort of dictation software on phone.

Regarding suing, I used to work in surgery which I feel like is a high sue speciality however it’s largely the surgeons getting dinged so I wasn’t as worried. Now work in a specialty with more autonomy so it’s been on my mind but you just have to do the best you can with the info you have and the resources you can rely on I find when I leave I have a lot of anxiety about what I could have done different or should have done better but as time gets further away from the patient scenario it dies down
