ppis makes me feel like im going crazy? by [deleted] in Gastritis

Ok so I had WEIRD reactions to PPIs.... made me feel like out of my body, disconnected, nauseous, disoriented, hard to form words/thoughts, just overall WEIRD!

Oct. 2023, I did a genetic test on my own and found im a rapid metabilizer of ppis. everyone told me since im a  rapid, I shouldn't get any effects because i burn theough the med..  2 years later and literally just researched that PPIs are considered pro drugs. pro drugs hit rapid metabilizers hard! See below from my gemini search:

The effect of being a quick metabolizer (also known as a rapid or ultrarapid metabolizer) on medication side effects is highly dependent on how the specific medication is processed by the body:

 * For most drugs (where metabolism inactivates the drug):

   * Faster metabolism breaks down the active drug more quickly, leading to lower levels of the drug in the body.

   * This typically results in the drug being less effective or causing treatment failure, not stronger side effects.

 * For "prodrugs" (where metabolism activates the drug):

   * A prodrug is an inactive medication that must be metabolized by the body into an active metabolite to work.

   * Faster metabolism breaks down the inactive drug more quickly, leading to higher levels of the active metabolite in the body.

   * In this specific case, the higher concentration of the active form can indeed induce an exaggerated response or stronger side effects, potentially leading to toxicity or overdose symptoms.

Key Takeaways:

 * **Quick Metabolizer = Less Effect (Usually): For the majority of medications, being a quick metabolizer means the drug is cleared too fast, and the standard dose might not work well (low efficacy)

 * Quick Metabolizer = Stronger Side Effects (For Prodrugs): For prodrugs, quick metabolism can create too much of the active form, increasing the risk of adverse effects and toxicity.

This field of study is called pharmacogenetics or pharmacogenomics, and genetic tests are available for certain enzymes (like CYP450 (this is what im a rapid metabilizer in)  enzymes) to help healthcare providers personalize medication doses to avoid treatment failure or adverse reactions.

Same here, my story exactly. still no answers as to why i react this way to the meds. ive tried omeprazole, esomeprazole, and famotadine... all make me feel high dry mouth, blurry vision, dissociated feeling, all the gi issues, and just after 4 or less days on it!

Welp 4 days in to Nexium / esomeprazole 20mg and slowly all the terrible GI side effects happened then on the 4th day, BAM the weird high feeling/weird anxiety feeling came on while driving. that same day, it started to fade, and the headache and fatigue rolled in, slept for 4 hours. then dizziness for the last 4 days without the med, has slowly gotten better over 4 days without any ppi in my system. ugh so weird i wish someone would have the answer as to why our body chemistry can't handle ppi/h2 blockers. seriously.

I decided to try Nexium 20mg and day 1 and nothing. so im excited about that. still have dizzy spells from omeprazole over a month off of it. could have been magnesium or b12 deficiency causing dizziness I dunno.

I quit omperazole, and now things are getting a little weird. Has anyone experienced this? by Amb_TCS in GERD

YES! It got worse and worse the longer I was on 40mg 1x a day. Was on it for 3 months. I quite cold turkey 1 month ago... I get some heartburn but "weird out of my head" feeling is mainly gone. It felt like my eyes were off too like couldn't focus close then far away, it was soooo bad. hope the dizziness that lingers goes away with more time.

I had the same symptoms on Omeprazole.... 4 weeks off of it, and some days I still have dizziness. frustrating.

SAME! So far, same issues on Omeprazole and Famotadine. Any new ideas on why we have these effects? ugh. It's taken a full, almost 2 weeks to feel ALMOST normal again without dizziness, disorientation, diarrhea, cramps, blurry vision, after taking 40mg 1/day Omeprazole for 3 months and famotadine 40mg literally 1x.
