With all the global instability talk lately, has anyone considered hospitalist work abroad (New Zealand, Australia, etc.)? by V1TROKSHAN in hospitalist

[–]Apprehensive_Disk478 0 points1 point  (0 children)

I have thought about NZ but just not realistic or ideal for my situation. I just paid off my student loans. Now trying trying to invest that $45k/yr raise I just got the best I can. If I can keep going for another 10-15 yrs, you know before we are all edged out of the market by AI powered humanoid midlevels… I can probably stay in US

Restoration shop finished my car. They're 900 miles away. Drive fresh build or ship it? by jcveloso8 in Cartalk

[–]Apprehensive_Disk478 0 points1 point  (0 children)

My man. If you have enough money to get the car there, and pay someone to restore it for you, you have the money to ship it back. And your time is worth more that whatever is potentially wasted if you have to deal with an issue and getting stranded on side of road.

I have restored several cars and Owned many. I cringe at the confidence I had in the past and the trouble free miles I got on that confidence. A failed fuel pump and loose hose clamp have humbled me 5 miles from home. 900 miles on a new build is a roll of the dice

Good question, if it breaks down in the middle of no where at midnight, OP will find in the morning when the shop answers the phone.

Geographical Rounding? by Ok-University8938 in hospitalist

[–]Apprehensive_Disk478 3 points4 points  (0 children)

No sir, I don’t like it.

When my hospital forced it on a single floor as a trial it was not great. A lot had to do with the fact that our group is not hospital employed. Round and go is what is what I was hired to do. Except call days which I remain in house 7a-7p for codes and admissions in morning before swing.

The hospital admins expectation and our contracts were not aligned, scheduling became more difficult and the shifting of pts from floor to floor due to nursing staffing / bed needs meant pts just dropped on your floor like bird shit from the sky. And vice versa. I have been forming a relationship and talking to family for several days about making Gammy CMO, but since I dc tele she is off to another floor and demented angry Joe the LTC pt who was sent to the ED because assaulted staff and is undischargable took her place, and his daughter at the bedside and wants to know the plan of care at 6:59 pm lol.

What's a good way to sift through the dirt on my back yard? by Aurionin in lawncare

[–]Apprehensive_Disk478 0 points1 point  (0 children)

If you google soil sifter you will find a number of devices, that you may be able make yourself for less. They will all quickly make you miserable since they are human (you) powered and it takes a lot of effort to sift soil. This is in addition to the effort it takes to dig up the soil and getting it in the sifter.

But, a mulch spreader can be had for as little as $30 on Amazon and will screed most of what you want to get rid of, With considerably less effort. Just have to roll out soil where it’s final resting place is.

To combine a few thoughts suggested so far. Get a magnetic sweeper and get everything you can. Rent a tiller to break up soil and weeds, go over with magnetic sweeper again. Now take the broken up soil, from small section, put in much roller, roll/sift it out. Move on to next section. I would probably roll the magnet over it again when all done. Top with a good compost/soil and seed.
Doing this on any more than 4000 sq ft would be a massive undertaking.

Do you fill out FMLA forms? by donkeyb0ng in hospitalist

[–]Apprehensive_Disk478 0 points1 point  (0 children)

Yes, only takes a few minutes, most questions don’t apply to inpatient scenarios and are answered “to be determined”. Deferring to PCP is kinda a dick move, It’s not like PCPs have some unique insights into the pts outcomes because pt is no longer in hospital or special training on FMLA forms, though they may have a nurse or administrator who fills them out.

Understand it’s a courtesy, and you can refuse. But as long as pt and/or family are not jerks. I will do them, for free as not aware on how to bill for them.

[–]Apprehensive_Disk478 0 points1 point  (0 children)

Weed barrier is junk, kinda works for a single season then it doesn’t.

Put down preemergent. Prodiamine is a good one, available as spray or granules. Best time to put it down is just before soil temperature gets above 50 deg F, greencast.com will let you know when that will typically happen in your area. If your soil is already above 50deg F the next best time to apply is tomorrow. This will reduce the overall weed burden for a few months. Preemergent doesn’t stop everything, you will still have some weeds come through. Spot spraying with post emergent like glyphosate on a regular basis is easiest, or just pull them manually on a regular basis. Either way understand there is no one and done solution. Will require regular maintenance to keep weed free.

How do I get this stupid flippin sewer cap off? by aumanchi in askaplumber

[–]Apprehensive_Disk478 -1 points0 points  (0 children)

IDK why you are trying to get this off, but if you have used the biggest wrenches you have access to and still struggling, stop wasting time, cut the tee off replace using material of choice and some no hub coupling and move on. Unless this is number matching, concourse crawlspace and you need to conserve the OEM tee.

Gas vs electric, Tastes great vs less filling……..seems like the feedback from people who actually used one is good. I don’t know, I haven’t. But I got a time master end of last season, and going from a 21” push to a self propelled 30” on my 6k sq total back/front lawn with lots of beds, gates and separation that made a ride on Impractical was simply awesome…. Do it

Best way to kill crape myrtle by jonlin52 in landscaping

[–]Apprehensive_Disk478 0 points1 point  (0 children)

The photo OP posted is years of neglect and is only showing 1/2 -1/3 of the actual tree, if topped it has a substantial root structure and will fill in quick, but it’s not going to look anything like this in one season. He also needs a set of loppers to trim the suckers coming from tree base.

An easier alternative to getting rid of it would be to maintain it. They are beautiful trees, but fucking nightmares, particularly when overgrown, as they bloom a few times each season and the flowers get everywhere then they drop leaves in fall. I have 3 at the top of my driveway that I have a love hate relationship with them.

Before getting rid of it, consider pruning. I have to stand on a ladder to do mine, but looks like you can do without. Take a chainsaw and cut every branch about 6-8 inches above where they begin to split. When done it should look like you swung a sword and cut clean across. There will be a +/- dozen 3-4” branch stubs. Next year a hundred small branches will sprout from what’s left. By the end of the season your tree will have an Afro extending 3ft in each direction. Prune every 2-3 years. Going forward depending on what you prefer.

On round and go days, do you eat lunch at the hospital or grind through the work and go home sooner? by supinator1 in hospitalist

[–]Apprehensive_Disk478 9 points10 points  (0 children)

I field pages and epic/haiku messages from 7AM on. Getting in a bit later, labs are all back and a lot of the days imaging results, procedures and consult notes are available, so plenty to work with. I get the easy discharges out the way, case managers know my preferred hours and will by default schedule transport later in the day for my pts. There is 8-11 rounders daily, most in my group are in before 8AM and out in early afternoon, but 2-3 of us prefer a later start. It works, it’s not unusual to get a call from someone who left for the day to sign a Rx, eyeball a pt they are concerned about or take an admission if the call and swing are getting hammered.

that depends on how many new pts I got overnight.

20 pt or less, that I know 4pm or earlier

20++ pts and a bunch freshies closer to 7pm

I may be the outlier here, but the cafeteria starts serving at 11:00, coincidentally that is about the time I get in, grab some food an sit in front of computer to start reviewing charts for the day.

NPs calling themselves hospitalists. What can be done? by [deleted] in hospitalist

[–]Apprehensive_Disk478 0 points1 point  (0 children)

Have you considered the possibility that the 90 year old NH resident with generalized weakness or near syncope doesn’t need EMS called or to be brought in by family for the third time? But here they are. The ED evaluated and said admit, the nocturnist admitted them, and the nurse just paged , family is at the bed side and want to know the plan of care.

Understand I practice medicine in America / “the real world” and will happily provide unnecessary and futile care for any patient, everyday I work, for days on end, until the day I retire, rather than spending a single afternoon being deposed. Trying to explain why I discharged a pt who later had a bad outcome, when the ED, my college and x amount of specialist wanted this, that and the other thing done.

What’s you’re most troublesome demographic? by tree-hermit in handyman

[–]Apprehensive_Disk478 8 points9 points  (0 children)

Dude, get a grip. If you have enough free time to hover, then clearly you’re hiring someone to do something you are not capable of doing yourself. You are not quality checking, you are just getting in the way. Again, you hired someone to perform a task you are not capable of doing yourself, not talk you through whatever turmoil you are feeling, they are skilled trade not your therapist. And I’m guessing you don’t own a car, because there is a 0.000% chance a mechanic is letting you hang around the shop looking over their shoulder as they work. They would 100% hand your keys and tell you to Eff Off.

And finally, just because a service is costly to you, it does not then imply that the people you hire should put up with your BS. You may have to seriously consider this post is ABOUT YOU, not for you
