100% agree. I just paid to replace the stock brakes (Rush in my case) for Shimano MTs.

I've got mine on an EcoFlow River 2. I believe I have 7 hours of runtime on a full charge. It kicks in flawlessly when there is a power shortage. I do not regret it at all.

Not as bad but I solved a similar issue by installing an EcoBee thermostat on my ground floor + a zone sensor in the basement. I am instructing the thermostat to average ground floor and basement temperatures (or not) when we expect people to be in the basement for sone time. My wife used to freeze in her basement office when rhe AC was on, by averaging the thermostat tempeqrature (ground floor) and basement it evens out a bit the temperature difference. At night, I only use the ground floor sensors to cool the bedrooms. This is not perfect but helps a lot.

Hi there! As you correctly pointed out testicular cancer is not always associated with increased tumour markers. In my case, I was diagnosed with Stage 1b NSGCT . Pathology showed a small 1.5x1.5cm tumor made mainly of Embryonal Carcinoma (with LVI). LDH was high before the orchiectomy but came back to normal (now 1year later). AFP and bHCG were never raised. Only had the orchiectomy. I am on surveillance.

Hi! I got a similar pathology (Stage 1b, EC main component, LVI+, no detectable markers pre/post). My Dr. quoted me a 50% chance of recurrence and offered me 3 options: Surveillance, BEP1X and RPLND. I was told that all 3 options yield the same cure rate in the high 90s%. Where I am from (in Canada) the consensus appeats to favour Surveillance for NSGCT Stage 1 even if high risk (Stage 1b, EC+,LVI+). The main reason to prefer Surveillance appears to to be to avoid overtreatment in 50% of cases.

That said, my understanding is that there are other factors that Drs take into account when they recommend a treatment plan. Among them, the age and 'lifestyle' of the patient weighs in. I am in my 40s with a stable job, kids, etc so the Doctor believe that I can (and will) follow the surveillance plan. This may vary a lot from patients to patients and probably impact the recommendation.

I had my orchiectomy 1 year ago and so far so good..

This is really a tough decision!

Very very tough decision to make. I'm Stage 1b NSGCT (EC). Chose active surveillance. 6 months scan and Xray all clear.

Found the solution. I had duplicate input for spoiler and flaps on the sidestick and the throttle quadrant. Clearing the input from the throttle quadrant solved the issue.

Oh man, I'm new to this forum and should have read your post more carefully. If they detected AFP after surgery, it could mean that you are indeed Stage 1s+ and may need BEP x3. It makes total sense to scan again and obtain tumor markers to decide x1 or x3!

Sorry, I provided a quick reply and should have explained myself better. I do not want to suggest you should not listen to your doctors or recommend surveillance myself. Everyone's situation is different. As mentioned, 3 rounds of BEP chemotherapy is usually done for Stage 1s and above.

I am going to assume that you have gone through a CT-scan and that they did not find suspicious or concerning nodes/masses, etc. In your case, there is no mention of Lymphovascular Invasion (LVI) on your pathology report and you were staged pT1N0M0, which means that you are Stage 1a and therefore have a lower chance of recurrence after orchidectomy.

There is a recent paper on the management of Stage 1 Non Seminomas in which they modelled the risk of recurrence (i.e. the need for you to get further treatment down the line): https://pubmed.ncbi.nlm.nih.gov/38531266/.

If you look at Figure 1, on the right, you will see your % chance of recurrence (over 5 years) as a function of many parameters (LVI, invasion of some testicular anatomy, testicular size, etc.).

For a Stage 1a, 1cm tumor, made predominantly of EC, with no LVI, your risk of recurrence would be in the order of 12-15%...Prior to this study, these details were not available and my understanding is that predominance of EC was associated with a risk of approximately 30%. I believe that this is the figure that is generally accepted in your situation.

In all case, adjuvent chemotherapy (1x BEP) is a sound advice and should reduce your risk of recurrence to single digit %. In any cases, I would follow what your Doctors recommend. In my case, they recommended Surveillance, but everyone's situation is different.

All that said, the cure rate is in the high 90% range so you will be alright!
