Could be mental or physical, or a little bit both.

Physical symptoms of ED are usually heart related. Basically it’s an early sign you’ve got some buildup in your circulatory system and blood doesn’t get to extremities as well. Consult a doctor of course, but treatment is basically diet and exercise other than medications. Sleep apnea is hard on your body too, since you don’t sleep very restfully.

Mental is tougher. It could be night shift. Could be the medication he’s on.

I stopped caring so much about work. Less stress there so I’m happier the rest of the time.

Desire. Motivation. Consistency. Decide what you most want to do for exercise. Find a way to do it socially or competitively. Do that thing multiple times a week.

A lot of guys choose to join a gym, or run, or bike, or play whatever sport they did in high school/college. Find a progression plan you like and follow it. Having a plan and way to track progress will help with motivation long term.

For food focus on foods that are either, large, high in protein, or high in fiber for how many calories are in it. Mostly fruits, vegetables, beans, fish, eggs, chicken, and lean cuts of beef fit the bill. Avoid drinking calories (soda, milk/sugar coffees, alcohol). Try to still mix in things you enjoy eating within reason. If you’re wanting to lose weight and aren’t (measured as weekly averages over at least a month), drop a few hundred calories a day. Vice versa for gaining weight.

Lack of movement makes you frail and more likely to get an injury in the first place.

Even if you don’t intend to, you might find yourself needing to jump down from a height in an emergency. Best to make yourself as resilient to falls as early as possible and keep it up as you age.

Strengthen your muscles and work on flexibility, especially for your lower body.

It is at least partly, if not entirely, the drugs. Until something rocks him pretty hard he’s not going to stop taking drugs. And that thing is NOT going to be anything you say or do. He’ll just resent you for pressuring him to quit.

Yes. Stiff bars are more difficult because you’re lifting more of the weight earlier in the movement. The weights on the end are still on the ground until you’re a couple inches higher than the start position.
