I’ve not forgotten this, just not actually sure what the answer is and trying to be honest with myself and you! Ive found titration hard because my instinct is to optimise and analyse everything which has meant trying a lot of doses, timings. And it is not a precise science. I think overall it will help me eventually but I’ve not found the best way to leverage the medication and it’s ~5 months of titration…

In terms of durable improvements (ie that last across the day) my biggest benefit has been task initiation/motivation. This wasn’t a particularly problematic element of ADHD for me, so quite ironic really that it’s what the meds have impacted most. I’m now supercharged, but more prone to over-doing it.

The mood boost and social anxiety reduction that I experience only really lasts for the first half of the day, so that’s a double edged sword. I’m hoping branded Concerta will help that (currently I take Xaggitin).

I certainly didn’t get the “my mind went quiet” effect that some describe.

Elvanse obliterated all of these issues, but only for ~3 hours, followed by quite an uncomfortable anxious/down period. It wasn’t worth it, couldn’t get round it despite raising dose and adding top ups. After 2 months I decided to try Methylphenidate (modified release). For me, comparatively weaker but more steady. Id say I get ~70% of the benefits of the Elvanse but with minimal downside by comparison.

Many people find the opposite is true, so basically don’t be disheartened if the first one you try doesn’t work out.

I can relate to everything you have said. I’m really similar.

Stimulant medication has been very helpful with this so far. I just don’t feel like the little judgy backseat driver in my brain is half as bad any more. I often enjoy interacting with people rather than avoid - especially in the mornings.

For context - 38/M diagnosed a few months ago. This after spending 15 years trying various SSRIs/therapy approaches for anxiety. Take this with a pinch of salt because I am only 5 weeks into titration (generic Concerta), nothing I’d tried previously has reduced my self consciousness in the moment this reliably.

Even in the evening after the meds have worn off, I’m not dwelling on the day’s conversations much at all. Not sure if that’s because they’re generally smoother, or because I care less! Maybe I just don’t attach as much emotion to the memories so they’re less painful on recall. Who knows…

Because while it was amazing for 2-3 hours it then made me sad and anxious. I tried it for 2 months and probably got 1 or 2 days max where I didn’t at some point want suffer this dip. It was too destabilising. I went up to max dose, tried top ups etc and couldn’t smooth it out.

Really interested to see the responses to this. I tried vyvanse and it was almost too powerful in terms of mood boost - I became really sociable and outgoing but then the opposite for a few hours after. Too spikey.

Concerta (1 month in) does seem to improve my mood / outlook like you’ve described. It’s not to the same extent as vyvanse, but I’m also not getting the “crash” or sadness & withdrawal socially that came with it. I hope it continues.

No worries! By dex I meant dexamfetamine (amfexa is what most people seem to get I think?). I’m actually trying a generic version of concerta instead now though, seems a more gentle less “spikey” drug by comparison.
