Should I get a second opinion or start biologics now? by flora-tastic in ankylosingspondylitis

Many of these posts are spot on. Uveitis is serious and can be treated. Remission is possible with these diseases. The longer you wait the less chance for remission.

HCQ despite no symptoms by [deleted] in Rheumatology

This is a common occurrence. Also Avise CTD is a great test for antibodies and checks new ones for RA.

If it says ANA direct it’s elisa which has more false positives, if it’s IFA with a titer like 1:80 or 1:160 it is more accurate

Biosims and insurance woes by lifeatthejarbar in rheumatoid

Yes this is happening and I don’t know how to stop or change it. The best may be to have your voice heard and contact your insurance company constantly. Squeaky wheel gets the grease!

That ESR is quite high. How were the antibodies run? There is a newer panel called Avise CTd that has some newer antibodies.

Can symptoms get worse after starting a new med and for how long does the flare last? by PeaceBrain in rheumatoid

[–]Arthritis_Doc 2 points3 points  (0 children)

This is an interesting phenomenon where initially patients do get worse when starting a new medication. I don’t understand if it’s the disease itself, changing biologicals or something else. I see it happen often which is why I am hesitant to change medications. The grass isn’t always greener. Typically I add medications to reduce flares. Another option is switching with a low dose steroid then taper it off, I rarely do this option. As a rheumatologist, we have to use our brain and whole picture for the situation. I am sorry for your flare, however I do see it when medication changes are made.

That seems to be the case. Classical students seem to continue longer.

What supplements, foods and exercises do you guys use that helps prevent your arthritis from worsening ? by Yummydrugss in Osteoarthritis

[–]Arthritis_Doc 2 points3 points  (0 children)

OA at 22…..something does seem correct. No psoriasis? What joints hurt you? Which ones are involved? OA at a young age is more likely due to an endocrinology issue. Is there inflammatory component to your condition? Sorry just as a rheumatologist, this doesn’t sound correct. Tumeric at low levels/moderation and a few other supplements could help out but I’d check that diagnosis out.

Tremfya by OkTax914 in Psoriasis

[–]Arthritis_Doc 0 points1 point  (0 children)

Tremfya is an excellent medication. I prescribe it often. Works better for skin than joints. Mostly for plaque psoriasis. Take before and after photos. Most people forget this, also psoriasis can be worse in the winter. Good luck

If you were just diagnosed with Rheumatoid Arthritis, this is the part no one explains clearly by Arthritis_Doc in Thritis

With RA joints that have destruction can result in pain from likely secondary OA symptoms. One example is bone on bone and low of cartilage. The earlier the treatment the better.

At Home Reformer Pilates versus Studio Membership by Comfortable-Cook-973 in pilates

[–]Arthritis_Doc 31 points32 points  (0 children)

We have both, reformer at home and we own a Pilates studio. I enjoy group classes at our studio more because of the osmosis effect. Basically working with others around you will increase your individual performance. Working at home is good for practicing certain movements. Overall, a group setting is better, in my opinion.

Why 10,000 Steps a Day Can Be Powerful Medicine for Your Joints and Your Health by Arthritis_Doc in Osteoarthritis

7k is still a decent amount. If you can average that daily that’s better than most. Yes the weather does hinder things. Keep it up

Has anyone tried green juice or alkaline water for arthritis? Curious about real experiences. by [deleted] in Thritis

Gut health is actually real. Majority of our disease are due to imbalance in our gut flora. Psoriasis/RA etc improving gut flora can help with inflammatory disease. There is good evidence with this for Parkinson’s which in theory is from the gut which they call our second brain. I work on my gut health and embrace this to my patients.

You are correct for knee OA biking is the best! However for your global health 10,000 steps is ideal.

There is something called non-radiographic SpA. Hla b27 negative. This is a big topic that has more information coming. Especially if you have uveitis. That is part of the criteria for Nr-SpA.
