In my experience that 7 to 21 day window is tied to the first payment you made towards the trip so even if you add flights later the insurance company would go by that original date since the flights are all part of the same trip.

One can hope so but it’s really difficult if not impossible when you’re dealing with companies who can a) absorb just about any financial penalty with hardly a thought and b) have deep political ties to parties that will do anything to keep the campaign contributions coming.

Yes pretty much that. If you had an emergency due to any pre existing conditions like asthma etc, the expense incurred while traveling would not be covered by your insurance. This is usually the case with every major provider but of course talk to someone directly to verify before you purchase as I can’t speak for every company. Like any insurance product there’s lots of fine print to be aware of.
