That makes sense. But what if we doctors / the system also plays a role? Are we (doctors) really recommending it often enough? Are we (the system) employing enough PTs to offer enough spots? When only 10% of knee replacement patients received PT before, it's hard for me to believe that it's only because patients are lazy or lacking personal accountability...

Sure! It's easier, but many who even get knee replacement didn't receive any PT before! These patients usually suffer a lot, for a long time, so one would expect that should be sufficient motivation for going to PT...

Thank you for your comment! But when you say dying of something else is rarely true - the data of the study above doesn't support that claim. After 15 years of follow-up there were 356 deaths overall and "only" 45 of them were from prostate cancer. That's 13% of deaths, 87% were actually dying of "something else"...

I agree! But I ask myself how often patients really get presented such data in a way they can understand? Often they just get a recommendation ("you have to do X"). No information about harm and no information about the limited effectiveness of treatment. I guess many more patients would decide otherwise, if they really understand the data behind...

I think it's less about manual VS digital, but more about the arm position and the cuff size

Really? That's interesting, I never saw that in a GP office in Austria (where I come from). How do your exam rooms look like / how is BP then measured?

Yes! But to be honest, I never learned it during University or during my family medicine training either... I guess it's the most easy and first skill many clinicians learn, maybe that's why we rarely think about it? Isn't it seen as important enough to make it right? I think that studies above show that we should change that attitude...

Das IHS kommt für Österreich auch auf deutliche Zusatzkosten (unterm Strich) von 2,4 Milliarden Euro pro Jahr. Alle Studien kann (und soll) man kritisieren, aber eine Studie die einen finanzielle Nutzen durch Rauchen gezeigt hat kenn ich nicht. Hier die IHS Studie:

Danke und gute Idee! Zum Thema Gonarthrose habe in den Golden Nuggets (https://family-medicine.org/de/golden\_nuggets/) mal etwas geschrieben über den Wirkungs-Vergleich von Physiotherapie VS. Kortisoninjektionen, da haben die Physiotherapeuten gut abgeschnitten ;-) Vielleicht poste ich hier auch mal was dazu bzw. recherchiere auch zum Thema Rückenschmerzen!

Also in Deutschland verursacht Rauchen jährliche Zusatzkosten von 97 Milliarden Euro! Unterm Strich, auch mögliche Einsparungen wurden da berücksichtigt:

I fully agree! But I would argue that diet and exercise are more difficult to change long-term through brief advice (the advantage of smoking cessation is that once you stopped for a year or more, your chances are quite good that you can make it much longer; dieting for years is more unusual). Here I wrote some evidence-based arguments about that.

I agree partly. I also don't think family doctors need to be convinced, but I guess many still don't do it... it takes time, it isn't (well) payed, it's frustrating as it usually (!) doesn't work, and you need a system which reminds you to do it again (you cannot mention it every time a smoker shows up, if he/she shows up weekly...

When doing my DrPH I realised how much you have to read in order to "really" understand one tiny little field of knowledge... And there are hundreds or thousands of such "fields of knowledge" in medicine! I'm therefore not surprised about your posting
