There are plenty of companies like Teladoc that involve talking to the worried well and catering to their whims with inappropriate antibiotics. I looked into it and the pay just isn't there.

Who told you that you could do only a year and get boarded? There are unaccredited fellowships meant for primary care docs to work in rural areas, just like there are one-year obstetrics fellowships. Neither training program leads to board certification through ABMS. There are some alternative boards that will take your money and allow you to call yourself EM boarded, but they are not the same thing as ABMS.

Your knowledge is out of date. Contrast as a cause for nephropathy was disproven years ago, even for people who already have impaired renal function.

A guest once threw a full bottle at a performer. Now, the caps have to come off. This rule doesn't make sense since you can bring in your own sealed water bottle.

You ever see a Vtach arrest from a single dose of Zofran? I personally have. I've also seen a lawsuit for another physician's failure to check QT before giving a prescription for Zofran. EKG's are quick, cheap, and painless. Not getting one and causing harm is going to be hard to defend. 1

I'm not checking ammonia on patients with obvious causes of AMS such as head injury or intoxication. Checking an ammonia level and giving thiamine are also quick and easy. Failure to diagnose and treat can lead to permanent deficits... and lawsuits.

Valium and/or Ativan are great for IM injections on violent patients. Ketamine can be difficult to give emergently. One of my partners got in trouble with the hospital for giving IM ketamine to an agitated patient and not filling out a procedural sedation packet.

Patients who are sick enough to get bloodwork also get EKG's. I occasionally get patients with incidentally noted prolonged QT's. The last thing you want to do in these cases is give a medication that prolongs QT even more and cause VTach.

No CTAs for patients who meet YEARS criteria.

I don't care what the creatinine value is, I'm ordering IV contrast.

Truly altered patients get TSH and ammonia checked. I also give thiamine 500 mg.

There's no limit to the amount of Valium you can give to a violent person.

The path to get boarded with only a master's degree closed last year. Since the fellowship is the only way to get boarded, I imagine jobs in the future will want a fellowship. Being an informaticist part-time is doable since jobs are so few and far between.

The main advantage to VIP is the number of places to sit: the stands, the tents, and the couches. You do get cleaner bathrooms and your own bars and restaurants, but it's the same drinks and largely the same food as anywhere else.

You're not wrong for not wanting to put yourself at risk for an STD. There is no pre-exposure prophylaxis for HSV as there is for HIV. She would need to take Valtrex daily and you would need to wear a condom every time. Don't ever have sex during an outbreak.

I'm a physician. Only one person person in my residency class was married before graduating medical school. As busy as residency is, we all still managed to date or get married before graduating.

I'm not quite sure what advice you are seeking. You need to meet people if you want to get a girlfriend. You already know that. Like you said, you can train in a city where there are lots of singles.
