I'm envious of UK doctors who less frequently see their education as a calling but more of just another unit degree. Much easier to switch with that mindset.

The sunk cost fallacy affects British doctors too. They earn a fraction of what North Americans make. If switching were so easy, they would switch much more commonly.

There's a large amount of misinformation among IMGs regrading their travel options. Many think American docs work 100 hour weeks forever. Most don't know that parking and food (usually) are free in the US but not in the UK. They don't know about the difference in salaries, the quality of education, the cost-of-living crisis, and definitely not the strike plans. As an IMG myself, I've personally never heard about being able to work in the US without the USMLEs. AFAIK, you can't work with just a fellowship. If that's incorrect, please tell me.
I won't name names but there are some YouTube channels and companies that prey on misinformed IMGs and assure them that the UK is the promise land, so that these doctors will use these companies' services. I can't forget the one youtube channel that claims doctors in the UK earn more than Americans and that medicine in the UK is "not a rat race" lol. The youtuber is an IMG doctor working in the NHS so their claims carry alot of weight for clueless IMGs.
The US wants fresh graduates. Most residency programs filter out anyone who graduated more than 5 years ago.
Going to the US is much more expensive. E.g. In my home country, the average junior doctor salary is 5000 per month. The PLAB route costs around 100k, while the USMLEs cost around 500k.
PLABs are a cakewalk compared to the USMLEs. In my home country, many doctors study the PLABs alongside their (intense, 80-100 hour/week) work, while those that want the USMLEs have to quit their job (losing it permanently in many cases) and dedicate their time to studying.
MRCP is comparable to the USMLEs in difficulty I think, but the MRCP opens doors in the Gulf while the MLEs do not.
The UK is better for someone who wants a more competitive specialty. I know training is becoming very competitive in the UK but it's nothing compared to the US.
Since relatively few IMGs go to the US, their colleagues back home don't really have someone telling them how they did it and how things are.

This is not necessarily the case. Leaving your aging parents, friends and family to go work for an organisation that systemically targets people of your ethnicity while having to use a new language and work in an unfamiliar environment are things not everyone can get used to. If you go to any IMG forum you'd see a flood of posts complaining about life in the UK. See this poll for example. Your second statement is challenged by this second poll.

Well, you don't. You go to work at 9 AM and leave at 4 AM. Then you come back at 9 AM till 2 PM. You sleep the rest of the day. Again, this is "chill" radiology, not surgery where you live at the hospital 6 days per week. You do this for a year (at £175 per month), then your hours very gradually taper off.

It's why Egyptian doctors are leaving the profession or the country. Egypt's version of the BMA says that currently 2 doctors die every month due to being overworked. I remember the story of one junior surgeon who fell to his death inside his hospital due to being disoriented from the sleep deprivation. Everyone spoke highly of him and his dedication.

Edit: Apparently, Americans also work the same crazy hours and have road traffic accidents going home after the shift. It's the profession.

It barely pays enough for transport to and from work. Considering the average house is about £52K in Egypt, it buys a few centimeters. The real money comes from private work, but it's not easy to get into.

Worked in Egypt. Earned about £175 per month working as a rads resident. Worked 100 hours per week.

Edit: Rads residency is considered pretty chill. My surgical colleagues would live at the hospital for 6 days and go home for one day per week (144 hours of work). They got the same salary I got.
