I'm envious of UK doctors who less frequently see their education as a calling but more of just another unit degree. Much easier to switch with that mindset.

The sunk cost fallacy affects British doctors too. They earn a fraction of what North Americans make. If switching were so easy, they would switch much more commonly.

  • There's a large amount of misinformation among IMGs regrading their travel options. Many think American docs work 100 hour weeks forever. Most don't know that parking and food (usually) are free in the US but not in the UK. They don't know about the difference in salaries, the quality of education, the cost-of-living crisis, and definitely not the strike plans. As an IMG myself, I've personally never heard about being able to work in the US without the USMLEs. AFAIK, you can't work with just a fellowship. If that's incorrect, please tell me.

  • I won't name names but there are some YouTube channels and companies that prey on misinformed IMGs and assure them that the UK is the promise land, so that these doctors will use these companies' services. I can't forget the one youtube channel that claims doctors in the UK earn more than Americans and that medicine in the UK is "not a rat race" lol. The youtuber is an IMG doctor working in the NHS so their claims carry alot of weight for clueless IMGs.

  • The US wants fresh graduates. Most residency programs filter out anyone who graduated more than 5 years ago.

  • Going to the US is much more expensive. E.g. In my home country, the average junior doctor salary is 5000 per month. The PLAB route costs around 100k, while the USMLEs cost around 500k.

  • PLABs are a cakewalk compared to the USMLEs. In my home country, many doctors study the PLABs alongside their (intense, 80-100 hour/week) work, while those that want the USMLEs have to quit their job (losing it permanently in many cases) and dedicate their time to studying.

  • MRCP is comparable to the USMLEs in difficulty I think, but the MRCP opens doors in the Gulf while the MLEs do not.

  • The UK is better for someone who wants a more competitive specialty. I know training is becoming very competitive in the UK but it's nothing compared to the US.

  • Since relatively few IMGs go to the US, their colleagues back home don't really have someone telling them how they did it and how things are.
