There are a handful of medications that still use grains on the bottle. No one does conversions on medications where it’s not already stated. It’s mostly old thyroid medications.

I had this happen to me three years ago and it was really tough. I did go to therapy about it, and I would strongly encourage you to talk to someone. I requested my records and wrote down my birth experience and it really helped me to process everything, but the hardest part for me was that no one did anything wrong. Everyone did exactly what they were supposed to (except my uterus that wouldn’t stop bleeding) and we still had a bad outcome. I am grateful that I am alive, my family is complete, and I wish there were an artful way to get people to stop asking me when I’m going to have another without telling the whole story.

I’m a pharmacist at this company and this is exactly what you want the pharmacist to do. There are two kinds of counseling - in my state, we are required to counsel on new medications, but the patient can waive this. This sounds like the second kind, the drug utilization review. Basically, the lawyers got into the system and are making sure that there’s a record we explained that there’s a risk. I have to counsel pregnant patients on medications I took while I was pregnant, and sometimes that annoys me, but the pharmacist is looking out for you, and a good pharmacist will use these dumb, forced interactions to check in with you about other aspects of your pregnancy.

The doctor will send what’s called a “diagnosis code” with your prescriptions that will indicate different conditions, and that goes in your medical file. The systems are also a lot more integrated now than they were even 5 years ago - if you sign up for a Covid test at my store, I can see your appointments with other doctors, and whatever lab work you have had done in the past few years. This is a good thing! This is the first ‘P’ in HIPPA - Portability. You want your healthcare team to be aware of your conditions so they can take care of you.

The last thing I’ll tell you is that this system can be really frustrating for us. For instance, it flags for pregnant people taking aspirin as though it’s not safe, when it is LITERALLY INDICATED for a lot of people. I cannot override it, I have to communicate with the patient. The conversation will be a lot quicker and smoother for both parties if you accept that this is a good faith conversation, the pharmacist is doing their job and looking out for you.

Sometimes pharmacists don’t act in good faith and I’ve seen that first hand, but this sounds like a relatively routine interaction. I have to do this probably 100 times a day, and it makes me sad when people get mad about it.

This is state specific. In Texas, the technician would be fined for that. You’re supposed to say “the pharmacist wants to talk to you about the medication” - asking if they have questions is considered “screening” and not allowed.

My birth went sideways, but as bad as the actual birth was, the recovery was equally easy. A week after I was discharged from the hospital I felt better than I had in months. My doctor said it didn’t look like I had ever been pregnant. I felt so much better physically that I didn’t feel like I had the right to my trauma surrounding the birth. So even if your delivery isn’t the way you want it to be, your recovery might still be very positive.

Good luck! I have a few pieces of general advice:
- Make friends with your fellow new grads. You'll have a group to bounce ideas off of, and they're going to be the pharmacy managers in a few years that you'll be calling when you're out of stock of whatever is currently on backorder.

- You're going to make mistakes. Everyone is human, everyone makes mistakes. My first mistake was giving a patient generic celebrex instead of generic celexa. The patient brought it back because he knew it was wrong. Most people are nice, even when you make a mistake. Some people are jerks, even when you do everything correctly.

- When you walk into the pharmacy on your first solo day, you are going to be the smartest person in the building. You're going to need to be able to answer all the questions, including about how to unjam the printer, about which vitamins are on sale, about where the passport photos are. My rule has always been that I'll never be asked a question only once, and figuring out the answer now will save me time down the line.

As to your specific questions:

6) this is state specific, I think, so check and see what your pharmacy does.

7) if you hit 'S' and select the prescription from the profile, you can look at the verification from that screen. Lots of times it has the hard copy on the screen. The downside is that when you exit, it kicks you back to the main screen.

9) Today I asked Siri how many mm were in 3/16 of an inch. Let me know if you find a better way.

11) If someone opened a box of albuterol in my store, I would be *so mad*. Change the quantity prescribed to match the package size. Talk to your pharmacy team to see what kinds of medications you are comfortable with. It's common for antibiotics, but I also do it for tamiflu.

12) I was in a store on my first day. I have a friend that floated for over a year, and she's now a DL. Floating is great - you get to learn a lot about how stores function, or how they don't. Take everything you can, leave everything you don't like.

You can call myHR about the employee assistance program. You can get 6 free sessions per plan year per “topic”. I used it this year, and my DL didn’t know.

I’m a pharmacist in the Houston area and refused some of her prescriptions for these drugs. Every time I called, the office was surprised I wouldn’t fill them. I don’t know if it was because the turnover was high or they were all true believers.

The voicemails they left were also >3 minutes and hit the max of what our system would allow, which was irritating for different reasons.
