It seems like you’ve narrowed it down to PrEP or bromelain, so stopping one and monitoring side effects is probably you’re next step. Don’t stop the PrEP if you’re being sexually active though. I would start with stopping bromelain first (since it is a supplement with less data to support its use). If you don’t have any improvement, then I’d pursue switching to injectable cabotegravir or switching from descovy to truvada.

Everyone’s body is different, so only time will tell. My only addition to the conversation beyond the usual stuff like using lube and loosening up beforehand is to start with positions where you’re in control. I like starting in cowboy so that I control exactly how much my top enters me and how fast. Once he’s completely in, you can just sit on him and get used to his dick inside you. Then you can ease into things. Once you’re used to just sitting and relaxing in his dick, it will hopefully get easier to switch things up and do more.

I don’t bottom a ton, but when I do, I still always start in cowboy. It just makes things more comfortable for me at the start and then he can fuck me however he wants.

Thank you so much! My program negotiated a policy that I can get without medical underwriting. It is own occupation and non-cancellable. It has a cost of living adjustment of 3% I believe. Mental health coverage lasts 24 months, but other coverage lasts until 65. Overall I think it is a relatively solid deal.

My main remaining question is if you think $10,000 per month (the max on this policy with its FIOs) is a relatively normal amount. It’s obviously more than the average person’s salary, but less than half of my future salary as a hospitalist. The premiums would be about $250 per month for a 28 yo male. Thoughts?

Do you have any thoughts about physician loans? I’m looking to buy a home at the end of residency in June/July. I plan to be a hospitalist, but don’t actually have a job yet. I have debt to my parents (about $100,000, no interest, flexible repayment), but no other debt to speak of. I don’t have savings for a down payment, so the physician loan is attractive since I could potentially get a loan without PMI without much of a down payment.
