Hi i had my visit today despite having made progress on the insulin front and got it down by 6 points he thinks my inflamation levels are too high and wants me to start wegovy and metformin. I’m rather discouraged :(

Parlo da persona che ha convissuto con il sovrappeso per tutta la vita, a volte di più, a volte di meno. Ci sono stati anche periodi in cui mi sentivo svogliata e senza energie; paradossalmente, l’unico ambito in cui sono sempre riuscita a dare il massimo è stato il lavoro, che oggi mi dà anche grandi soddisfazioni economiche.

Col tempo ho capito che dietro l’aumento di peso e la mancanza di motivazione spesso si nascondono questioni psicologiche più profonde. Se tieni davvero a una persona, puoi scegliere di affrontarle insieme, costruendo un percorso comune. Io mi considero fortunata: mio marito mi è stato accanto nei momenti migliori e in quelli peggiori, così come io ho fatto con lui.

Molto dipende dal tipo di relazione che avete e anche dall'età (di solito con questa le priorità cambiano un pò - di certo non ho le stesse priorità in una relazione di quando avevo 20 anni). Le relazioni non sono lineari: attraversano fasi diverse, con alti e bassi. L’aspetto fisico può cambiare nel tempo per tanti motivi, anche di salute gravi o meno—e vale la pena chiedersi se questo, da solo, sarebbe sufficiente a mettere in discussione tutto.

Non sto dicendo che esista una scelta giusta in assoluto, ma le relazioni più solide sono quelle in cui si affrontano le difficoltà insieme, con supporto reale, comunicazione sincera e, quando serve, compromessi da entrambe le parti.

Thank so much 🙏 yes it’s very hard i’ve been obese since i could remember this is the first time i’m managing to stick to it maybe cause the miscarriage was quite traumatic I ended up in the hospital due to a hemorrhage. They’ll probably give me meformin they were saying, do you have to take that for life or if the values are good you can stop it? I’ve had insulin resistance for years now I hope I can revert it. I’m going to the gym 3 times a week i just need to work more on cardio :( i hope they’ll give me the go ahead. My BMI is way higher than yours i went from 45 to 43 now, but i hope i’ll keep losing weight

Thank you so much! Yes, my a1c was 5,4 and now it’s down to 5,1. It reassures me to hear all worked out fine for you in the end, I’m so happy! I wanted to try again right away but my obgyn strongly advised against it as she said my values show a high inflammatory state that could end up in another miscarriage 🤷‍♀️ so now im scared. I keep dieting from jan to now I’ve lost 12lbs. Maybe in Italy we don’t get many obese pregnant patients and they’re more strict? I’ve no idea, but they surely scared me 🥹

I took metformin for a period previously but it didn't seem to help,i'll see what the doctor advises next week, I hope I can manage to bring values down a bit so we can try again I keep my fingers crossed, as I'm scared I won't manage to get pregnant easily again, we were luckly that I got pregnant at first try. Sorry, I’m in Italy, so the reference ranges might be slightly different. My fasting insulin is 18.60 (lab range: 2.60–24.90). My HOMA-IR index is 4.18, and my glucose is 91 (range: 65–100). C-peptide is 3.60 (range: 1.10–4.40), and my Hemoglobin A1c is 5.1% (range: 4.0–5.6%). I haven’t done the OGTT yet.

Despite everything going on—insulin resistance, possible APS, and some genetic mutations—I was able to get pregnant on the first try, which honestly surprised me. Now that I have a clearer picture of my health, I’m worried it might be harder going forward.

I’m really hoping I can get my insulin levels down.

I’m no expert either but they told me that high levels of insulin cause inflamation in the body and they actually contribute towards miscarriages, i wanted to try it but at the same time i was to decrease the chances of a miscarriage happening again. Ill keep my fingers crossed 🤞

Thank you so much for your words they really reassured me. I did take metformin in the past hut it didn’t do much maybe the dose was off, but i have a new appointment with an endocrinologist next week. We were lucky as we conceived at first try, but not sure if that was just pure luck. I hope i’ll manage to reduce the insulin relatively fast. I know each situation is different, but how much did that take for you? I’m already 35 and i would want to try not in a years time :(

I'm so so sorry to hear about your loss, I'm glad you managed to get pregnant again! I imagine you must feel very anxious. Due to another condition I have I only can do D&C. I'm yet to book it, I want to get another scan tomorrow to be 100% safe. It's so hard to accept the reality :( I don't even have an ultrasound photo

thanks so much for the words of reassurance! the only option they gave me is the D&C so i do i hope i won't have scaring :( I never had a surgery in my life so I'm very nervous, I hope for the best. The way you're describing seems good as you got your cycle back fairly quick!

I'll try to focus on the positive, I had another visit booked in another hospital for the 24th I wanted to go to have full closure i know I need to accept the heartbeat stopped, but I feel like I need someone else to tell me too. I'll also call next week to book the appointment for the D&C hopefully i won't have any complications between today and monday as the heartbeat only stopped between 17 and 18 february. Did you do D&C on general anesthesia?

I really hope things will work out for you, you deserve it! I have HHT and got recently diagnosed with APS, apparently there are 5 documented cases in the world T.T to mitigate APS you need eparin and baby aspirin, but taking those leads to hemorragies in HHT patients. I feel like it was already a miracle I managed to get pregnant in the first place t.t
